消瘰合剂Ⅰ号联合西药治疗颈淋巴结核患者43例疗效观察

摘　　要：目的观察消瘰合剂Ⅰ号在颈淋巴结核治疗过程中的作用。方法将60例颈淋巴结核患者随机分为治疗组和对照组各30例,对照组采用正规西药化疗方案,强化期用异烟肼片(H)+利福平胶囊(R)+吡嗪酰胺片(Z)+乙胺丁醇片(E),以上药物均口服2个月;巩固期用H+R,口服4个月。治疗组在对照组基础上加用消瘰合剂Ⅰ号,每日1剂,口服4个月。观察两组临床疗效,治疗前后记录两组中医症状评分,检测两组患者干扰素-γ(IFN-γ)及肿瘤坏死因子-α(TNF-α)水平,并比较两组毒副反应情况。结果治疗组临床疗效总有效率为86.4%,对照组为66.7%,治疗组明显优于对照组(P<0.05)。治疗组患者抑郁、纳呆、发热、乏力症状评分均较本组治疗前明显降低(P<0.05),治疗组治疗后纳呆、乏力症状评分较对照组明显降低(P<0.05)。治疗组治疗后IFN-γ水平较治疗前明显升高,TNF-α水平较治疗前明显降低(P<0.05),两组治疗后TNF-α水平差异无统计学意义(P>0.05)。结论在颈淋巴结核治疗过程中,消瘰合剂Ⅰ号能提高机体免疫能力、改善症状。Objective To observe the therapeutic effect of Xiaoluo Mixture Ⅰ for cervical lymphadenopathy（ CL）. Methods Sixty cases of CL were randomized into the treatment group and control group,with 30 in each.The control group was given routine western medicine. The treatment group was added with Xiaoluo Mixture Ⅰ one dose daily. The treatment lasted for 4 months. The efficacy was observed. The TCM symptom scores were evaluated before and after treatment. The levels of interferon-γ（ IFN-γ） and tumor necrosis factor-α（ TNF-α） were detected.The adverse reactions were compared. Results The total effective rate of the treatment group and control group was86. 4% and 66. 7% respectively,with a significant difference between groups（ P〈0. 05）. The symptoms such as depression,poor appetite,fever and fatigue were significantly relieved in the treatment group after treatment（ P〈0. 05）. Comparing with the control group,poor appetite and fatigue were significantly relieved in the treatment group after treatment（ P〈0. 05）. The IFN-γ level was significantly increased but the TNF-α level was significantly decreased in the treatment group after treatment（ P〈0. 05）. There was no significant difference between groups in the TNF-α level after treatment（ P〈0. 05）. Conclusion Xiaoluo Mixture Ⅰ could improve the immune capacity and symptoms in the treatment of CL.
