潜降颗粒联合西药治疗高血压病伴舒张性心力衰竭患者39例临床观察

摘　　要：目的观察潜降颗粒联合西药治疗高血压病伴舒张性心力衰竭患者的临床疗效。方法将78例高血压病伴舒张性心力衰竭患者随机分为治疗组和对照组各39例,对照组采用规范化抗心力衰竭西药治疗,治疗组在对照组基础上加用潜降颗粒治疗,两组均治疗8周后观察中医证候疗效。比较两组患者治疗前后N-末端B型钠酸肽(NT-proBNP)浓度以及超声心动图各指标[左房内径(LAD)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、室间隔厚度(IVSD)、左室后壁厚度(LVPWD)、二尖瓣口舒张早期及舒张晚期流速峰值比值(E/A)]。结果治疗组中医证候疗效总有效率优于对照组(P<0.05)。两组治疗后NT-proBNP浓度均较治疗前明显降低(P<0.01),且治疗组降低更显著(P<0.05)。与本组治疗前比较,两组治疗后LVEDD、IVSD、LVPWD及治疗组LAD明显降低、E/A明显升高(P<0.01);治疗组治疗后LAD降低更显著(P<0.05)。与本组治疗前比较,两组治疗后收缩压、舒张压及脉压比较均明显降低(P<0.05或P<0.01)。两组治疗后比较,治疗组收缩压降低更显著(P<0.05)。结论潜降颗粒联合西药治疗高血压病伴舒张性心力衰竭的患者,可改善患者临床症状及心功能。Objective To observe the therapeutic effect of Qianjiang Granules for hypertension with diastolic heart failure（ DHF）. Methods Totally 78 cases of hypertension with DHF were randomized into the treatment group and control group,with 39 in each. The control group was given routine chemical medicine. The treatment group was added with Qiangjiang Granules. The efficacy of TCM syndromes in both groups were observed after 8-week treatment. The levels of N-terminal pro-B-type natriuretic peptide（ NT-proBNP）,left atrial diameter（ LAD）,left ventricular end systolic diameter（ LVESD）,left ventricular end-diastolic diameter（ LVEDD）,interventricular septum diameter（ IVSD）,left ventricular posterior wall diameter（ LVPWD） and mitral ratio of peak early to late diastolic filling velocity（ E /A） were compared in both groups before and after treatment. Results The total effective rate of TCM syndromes in the treatment group was superior to that in the control group（ P〈0. 05）. The NT-proBNP level was significantly decreased in both groups after treatment（ P〈0. 01）,especially in the treatment group（ P〈0. 05）. The levels of LVEDD,IVSD and LVPWD in both groups and LAD in the treatment group were significantly decreased after treatment（ P〈0. 01） but E /A was significantly increased（ P〈0. 01）. The LAD level in the treatment group was significantly lower than that in the control group after treatment（ P〈0. 05）. The levels of systolic blood pressure,diastolic blood pressure and pulse pressure were significantly decreased in both groups after treatment（ P〈0.05 or P〈0.01）,especially systolic blood pressure in the treatment group（ P〈0. 05）. Conclusion The combo therapy of Qianjiang Granules and chemical medicine can improve symptoms and heart function of hypertension with DHF patients.
