基于病证诊断的2445例感染性疾病患者免疫特征研究

摘　　要：目的探讨感染性疾病患者的免疫特征。方法采用前瞻性、多中心、大样本、注册登记式研究方法,采集感染性疾病患者血清进行免疫学指标检测,包括环磷酸腺苷(cAMP)、白细胞介素1β(IL-1β)、白细胞介素4(IL-4)、白细胞介素6(IL-6)、白细胞介素8(IL-8)、巨噬细胞炎性蛋白1β(MIP-1β)、前列腺素E2(PGE2)、肿瘤坏死因子-α(TNF-α)8项指标,并分析不同原患疾病、发病天数、发热情况及不同中医证候患者各指标特征。结果共采集2445份感染性疾病患者免疫指标,IL-6、cAMP、PGE-2和TNF-α在不同体温层面差异具有统计学意义(P<0.05或P<0.01),且中热组偏高;除PEG-2外,不同原患疾病患者其他7项免疫指标分布差异均有统计学意义(P<0.01),且肺炎组各指标值略高;IL-8指标在不同发病天数分布差异有统计学意义(P<0.05),以发病1周最高;风邪组患者各指标值略高(P<0.01)。结论感染性疾病患免疫指标往往会升高,并且患者体温、原患疾病、发病天数、中医证候对其有一定影响。Objective To research the immunity characteristics of patients with infectious diseases. Methods A prospective,multi-center,large sample and registration method was used. The serums of patients with infectious diseases were collected and the immunological parameters such as cyclic adenosine monophosphate（ cAMP）,interleukin-1β（ IL-1β）,interleukin-4（ IL-4）,interleukin-6（ IL-6）,interleukin-8（ IL-8）,macrophage inflammatory protein 1β（ MIP-1β）,prostaglandin E2（ PGE2） and tumor necrosis factor-α（ TNF-α） were detected. The primary disease,disease duration,fever situation and different characteristics of TCM syndromes were analyzed. Results Totally 2 445 patients with infectious diseases were collected and their immunological parameters were detected. There were significant differences between different body temperatures in the levels of IL-6,cAMP,PGE-2 and TNF-α（ P〈0.05 or P〈0. 01）. The levels of IL-6,cAMP,PGE-2 and TNF-α were slightly higher in the moderate heat group. There were significant differences between different primary diseases in the distribution of 7 immune parameters except PEG-2（ P〈0. 01）. The immune parameters were slightly higher in the pneumonia group. There was a significant difference between different disease duration in the IL-8 level（ P〈0. 05）. The IL-8 level was the highest in the1 st week of onset. The immune parameters were slightly higher in the wind pathogen group（ P〈0. 01）. Conclusion The immune parameters in patients with infectious diseases are often increased and with a certain relationship between body temperature,primary diseases,disease duration and TCM syndromes.
