摘 要:目的:探讨血清甲状腺激素水平对于诱发排卵的多囊卵巢综合征(PCOS)患者卵泡生长的影响。方法:应用经克罗米芬+人绒毛膜促性腺激素(CC/hCG)诱导PCOS患者(89例)卵泡生长,根据卵泡生长情况分为卵泡生长正常组(49例)及卵泡生长障碍组(40例)。在排卵周期测定卵泡不同时间的大小及相应血清甲状腺激素(TSH)含量,对结果进行统计学分析。结果:卵泡生长的第0、5、10天,两组卵泡生长大小、T3和T4水平差异均无统计学意义(P均>0.05);卵泡生长第15天,卵泡生长障碍组卵泡大小〔(15.41±0.11)mm〕、血清T3〔(1.97±0.17)nmol/L〕、T4〔(97.43±0.16)nmol/L〕均低于卵泡生长正常组〔(19.18±0.19)mm,(2.69±0.15)nmol/L,(124.38±0.29)nmol/L,t=-0.886,-1.823,-1.745,P均<0.05〕,障碍组TSH水平〔(3.43±0.10)μIU/ml〕高于正常组〔(1.38±0.18)μIU/ml〕,两组比较,差异有统计学意义(t=-1.923,P<0.05)。结论:卵泡生长障碍组PCOS患者TSH水平较低,甲状腺激素水平低下在一定程度上影响卵泡生长。Objective: To explore the effects of serum thyroid hormones levels on follicular growth of patients with polycystic ovary syndrome (PCOS) treated by induced ovulation. Methods: Clomiphene (CC) and human ehorionic gonadotropin (hCG) were used to in- duce follicular growth in 89 patients with PCOS, then the patients were divided into normal follicular growth group (49 patients) and follicle growth disorder group (40 patients ) according to follicular growth situations. The sizes of follicles and the concentrations of serum thyroid hormones at different times during ovulatory cycle were measured, the results were analyzed statistically. Results: There was no statistically significant difference in the sizes of follicles, the levels of triiodothyronine ( T3 ) and tetraiodothyronine ( T4 ) at 0, 5, 10 days of follicular growth between the two groups ( P 〉 0. 05 ) ; on the fifteenth day of follicular growth, the size of follicles, serum T3 level and T4 level in folli- cle growth disorder group were ( 15.41 ±0. 11 ) ram, ( 1.97±0. 17) nmoL/L and (97.43 ±0. 16) nmol/L, respectively, which were statis- tically significantly lower than those in normal follicle growth group [ (19. 18 ± 0. 19) mm, (2. 69 ± 0. 15) nmol/L, (124. 38 ±0. 29) nmol/L] ( t = - 0. 886, P 〈 0. 05 ; t = - 1. 823, P 〈 0. 05 ; t = - 1. 745, P 〈 0. 05 ) . Thyroid stimulating hormone (TSH) level in follicle growth disorder group was (3.43 + 0. 10 ) μIU/ml, which was statistically significantly higher than that in normal follicle growth group ( ( 1.38 ± 0. 18 ) μIU/ml] ( t = - 1. 923, P 〈 0.05 ) . Conclusion: TSH level in patients with follicle growth disorder is relatively low, the decreases of thyroid hormones levels may affect follicular growth to a certain extent.
