摘  要:目的:利用微信平台这种多媒体形式,完善出院的糖尿病患者的延续护理工作,有针对性地为患者提供帮助,提高患者遵医行为,相关知识水平认知水平,自护能力和疾病控制程度,更好的为患者服务。方法:将80例糖尿病患者随机分为对照组和观察组,观察组患者出院后进行延续护理,对照组不进行延续护理,只给予常规护理,1年后收集患者在疾病有关知识掌握情况,自护能力,疾病控制程度等资料进行比较。结果:延续护理组在疾病有关知识掌握情况,自护能力,疾病控制均明显优于常规护理组。两者比较差异有统计学意义(P〈0.01)。结论:延续护理有效提高出院糖尿病患者的认知水平,自护能力,改善临床症状,提高生活质量。OBJECTIVE: To improve the continual nursing in the patients with diabeties mellitus by using multiple-media form such as WeChat in order to enhance the patients’ cognitive level and the degree of the disease control and so on. METHODS: 80 pa-tients were divided into control group and observation group. Observation group received continual nursing and control group accepted routine mursing. The meterials mentioned above were compaired after 1 year. RESULT: The cognitive level and the disease control and so on were better than those in the control group. There were statiscally significant difference between these two groups ( P〈0. 01). CONCLUSION: Continual nursing can improve these patients’ cognitive level, clinical symptoms and the quality of life.
