基于现代文献的灸法治疗感冒的临床证据研究

摘　　要：目的评价灸法治疗感冒的临床研究,总结与灸法临床实践高度相关的高质量证据,为临床策略的选择提供参考。方法以灸法临床实践中常见的重要问题为中心,全面检索目前可获得的临床研究文献,按照循证医学5级标准从高到低选用能够回答相关临床问题的证据,采用RevMan5·0·20统计结局指标。结果有25篇符合纳入标准的临床研究。B级证据证实灸法治疗感冒的有效性,化脓灸可以达到;相对常规抗感冒药,灸法可能有治疗优势或相当的疗效,如感冒清、先锋Ⅳ(C级证据)、荆防达表汤(C级证据)、维C银翘片(C级证据)等;仅有2项不同刺法灸法之间比较,1项为B级证据显示化脓灸比单纯针刺效果好,并在提高机体免疫较优;1项为C级证据表明温和灸比针刺效果好:4项C级证据表明灸法配合其他对比药物其效果要好或者效果相当,如雷火灸配合小青龙汤治疗感冒疗效优于西药等。而部分措施疗效和西药相当,比如壮医药线点灸配合佩药疗法对照达菲胶囊,但是其对症状改善更优等。结论笔者认为灸法治疗感冒是有效的。灸法治疗感冒与西药相比很可能等效甚至更优,当然灸配合其他措施的效果也有一定优势,但是尚需要更高质量证据来证实。Objective This essay is aimed to make clinical evaluation of moxibustion in the treatment of colds,and to make summary of high quality evidence that is highly correlated with moxibustion in clinical practice,in order to provide the reference for clinical strategies. Methods Centering on the important issues of common methods in clinical practicer,the writer made a comprehensive research of clinical literature currently available,to answer clinical questions related based on 5 levels of evidence-based medicine from high to low,with RevMan5·0 ·20 statistical software to calculate the outcome. Results 25 studies met the clinical research criteria. Class B evidence proves that the effectiveness of moxibustion in treatment of cold,festering moxibustion has the clinical effect; compared with conventional anti-cold medicine,moxibustion therapy may have the advantage or equivalent clinical effect,such as Ganmaoqin capsule,Cefalexin（ level C evidence）,Jingfangdabiao Decoction（ level C evidence）,vitamin C Yinqiao Tablets（ level C evidence） and so on; only 2 method of different comparison of needling and moxibustion,1 class B evidence to manifest that using festering moxibustion is more effective than only using acupuncture,and the former one is better than the latter one in improving the patient＇s immunity; 1 level C evidence to show that mild moxibustion is more effect than acupuncture： 4 level C evidence that moxibustion combined with other contrast medicine obtained better or equivalent clinical effects,such as thunder fire moxibustion combined with Xiaoqinglong Decoction got better clinical effect than that of Western medicine in the treatment of common cold. And some measures have equivalent clinical effect compared with Western medicine,such as the Zhuang medicine medicated thread moxibustion combined with medicine control Tamiflu capsule,but the former one＇s improvement of the symptoms is better.
