四川地区阻塞性睡眠呼吸暂停低通气综合征患者持续正压通气治疗依从性研究

摘　　要：目的探讨我省患有阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的患者进行持续正压通气(CPAP)治疗的依从性及其影响因素。方法选择在我院行PSG检查,诊断符合《睡眠呼吸暂停综合征诊治指南》且资料完整,拟行CPAP治疗的OSAHS患者151例,其中男119,女32例。记录其一般情况及PSG监测数据。制定依从性调查表,并进行电话随访。分析其治疗依从性及影响治疗依从性的因素。结果 1我省OSAHS患者治疗依从性好者为52.3%,经济因素是影响患者治疗依从性的一个明显因素;2在影响患者依从性的因素中,患者经CPAP治疗后主观症状的改善是一个主要的影响因素;3在四川,炎热天气因素是影响患者治疗依从性的一个较重要因素;4患者的性别、年龄、职业、学历及AHI、BMI、颈围等不是影响患者CPAP治疗依从性的因素;5CPAP使用中的副作用如压力不适、鼻咽不适、面罩漏气等对患者治疗的依从性无明显影响。结论我省OSAHS患者进行CPAP治疗的依从性好者为52.3%,依从性较国外患者差,影响患者治疗依从性的主要因素是患者治疗后的症状改善,在四川地区,经济因素及气温也是影响患者治疗依从性的原因。Objective To explore influencing factor and compliance with continuous positive airway pressure （ CPAP） in obstructive sleep apnea syndrome in sichuan. Methods 151 patients with obstructive sleep apnea syndrome diagnosed by means of polysomnwgram﹙PSG﹚were titrated in our hospital, male 119 and female 32,They are plan to by CPAP for treatment . Record their data of PSG and general state of health. we, telephone them about their therapy ,and record it. Analyses influencing factor and Compliance with CPAP in obstructive sleep apnea syndrome. Results ①The compliance with CPAP in OSAHS is 52. 3%in sichuan,poor is a predominant factor influenceing compliance;②Complaint improved by CPAP is a predominant factor among factor influenceing compliance.③Based on our research;we think sex age and AHI BMI size of neck in OSAHS patients are not factor influenceing compliance.④Hot weather is also a factoe infulenceing compliance in Sichuan 5 adverse reaction caused by CPAP, such as the pressure of CPAP、inward mask leakage nasopharynx complaint ,don’t effect compliance with CPAP in patiarnts with OSAHS. Conclusion The compliance with PAP in OSAHS is 52. 3% in sichuan, complaint improved by CPAP is a pre-dominant factor influence compliance, poor and hot weather are also importent factor influence compliance.
