多层螺旋CT对闭合性肠及肠系膜损伤的临床诊断

摘　　要：目的：分析研究多层螺旋CT对闭合性肠及肠系膜损伤的临床诊断价值。方法回顾性分析27例肠及肠系膜损伤患者的临床资料及CT表现。结果27例患者中，CT显示腹腔游离气体16例，造影剂外溢2例，肠系膜增厚8例，肠系膜血肿4例，肠壁血肿3例，腹腔内游离积液征5例，肠系膜损伤合并肺、肾挫伤各1例。结论肠及肠系膜损伤的CT表现有一定的特征性，是早期诊断闭合性肠及肠系膜损伤的重要手段。根据CT征象可较准确地判断手术指征。Objective To investigate the diagnostic value of spiral CT in the blunt bowel and mesenteric trauma.Methods A retro-spective analysis of CT data was conducted in 27 patients with blunt bowel and mesenteric trauma who were admitted in our hospital .Results Among the whole patients,peritoneal cavity free air in 16 cases,contrast agents spillover in 2 cases,mesentery thickening in 8 cases, mesentery hematoma in 4 cases,bowel wall hematoma in 3 cases,intraperitoneal free fluid in 5 cases,mesenteric injury with pulmonary and renal contusion each in 1 case.Conclusion Spiral CT has important value for diagnosis of blunt bowel and mesenteric trauma,which can make an accurate preoperative diagnosis.
