抗雄激素联合双侧睾丸切除术在前列腺癌患者中的应用

摘　　要：目的探讨抗雄激素联合双侧睾丸切除术在治疗晚期失去根治手术机会前列腺癌患者中的应用。方法收集我院2007年3月至2013年3月收治的80例失去根治手术机会的前列腺癌患者,随机分为观察组与对照组,观察组使用抗雄激素联合双侧睾丸切除术治疗,对照组使用常规药物去势治疗。比较两组患者生理、心理、社会功能、临床症状、不良反应和生活质量之间的差异。结果观察组患者躯体功能、认知功能、情绪角色功能和社会功能4项得分分别为78.79±5.49、75.40±5.37、78.41±6.50、71.19±5.43,对照组分别为71.19±5.32、75.40±5.37、71.10±8.54、65.09±5.48,观察组上述4项得分均显著高于对照组(P<0.05)。观察组患者疼痛、恶心、呕吐和疲倦得分分别为51.11±4.32、43.21±7.70、42.11±8.23、58.40±12.12,对照组分别为67.60±10.13、54.30±7.60、50.48±9.21、68.54±3.89,观察组上述4项得分均显著低于对照组(P<0.05)。观察组患者人均报告不良反应次数(3.21±1.13)次,显著低于对照组的(5.49±2.98)次(P<0.05);观察组患者生活质量评分为76.50±11.80,显著高于对照组的62.29±12.18(P<0.05)。结论抗雄激素联合双侧睾丸切除术可以有效提高晚期前列腺癌患者的各项生理、心理和社会功能,降低患者临床症状和不良反应发生情况,改善患者生活质量。Objective To investigate the effects of anti-androgen joint bilateral orchiectomy in the treatment of prostate cancer patients.Methods 80 prostate cancer patients were selected from March 2007 to March 2013 in our hospital.Patients were randomly divided into observation group （anti-androgen combined with bilateral orchiectomy） and control group （conventional medical castration）.The physical,psychological,social function,clinical symptoms,adverse reactions and quality of life were compared between the two groups.Results The physical function,cognitive function,emotional role function and social function scores in observation group were 78.79 ± 5.49,75.40 ± 5.37,78.41 ±6.50,and 71.19 ±5.43,which were higher than those of control group（71.19 ±5.32,75.40 ±5.37,71.10 ± 8.54,and 65.09 ± 5.48）,there were significant differences between the groups （P ＜ 0.05）.The scores of pain,nausea,vomiting and fatigue in observation group were 51.11 ± 4.32,43.21 ± 7.70,42.11 ± 8.23 and 58.40 ± 12.12,which were lower than those of control group（67.60 ± 10.13,54.30 ±7.60,50.48 ±9.21,68.54 ±3.89）,there were significant differences between the groups（P ＜ 0.05）.The number of patients reported adverse reactions in observation group was significantly lower than that of control group （3.21 ± 1.13 vs.5.49 ± 2.98,P ＜ 0.05）.The score of life quality in observation group was significantly higher than that of control group （76.50 ± 11.80 vs.62.29 ± 12.18,P ＜ 0.05）.Conclusion Anti-androgen combined with bilateral orchiectomy can effectively improve the physical function and life quality of prostate cancer patients,and decrease clinical symptoms and adverse reaction.
