类风湿关节炎患者血脂水平异常的临床意义

摘　　要：目的:探讨类风湿关节炎(rheumatoid arthritis,RA)患者病情活动程度与血脂水平的关系。方法:收集2010年12月至2012年12月包头医学院第二附属医院确诊的140例RA患者,根据DAS28分为A组、B组及C组。结果:处于活动期患者血清TG、LDL水平明显高于对照组(P<0.01),HDL水平低于对照组(P<0.01),活动期RA患者的ESR、C3、C4、CRP、抗CCP、RF、IgA、IgG、IgM、TNF-a高于非活动期。血清TG、LDL水平与ESR、CRP、TNF-a呈正相关,血清HDL与ESR、CRP、TNF-a呈负相关。结论:活动期的RA患者体内存在脂质代谢明显异常,除ESR、CRP外,血脂可能成为评价RA病情活动的指标,作为评价类风湿关节炎疾病严重程度的指标之一。ObjectiVe：To inVestigate the relationship between disease actiVities and the serum lipid leVel in patients with rheumatoid arthritis（ RA）. Methods：140 cases of RA,diagnosed from December 2010 to December 2012 in the Second Affilia-ted Hospital of Baotou Medical College,were diVided into group A,group B and group C according to DAS28. ResuIts：The leV-el of serum TG and LDL in RA patients at actiVe phase of disease was significantly higher than that in the control group（ P 〈0. 01）,with HDL leVel lower than that in the control group（ P 〈0. 01）. The leVels of ESR,C4,CRP,C,anti-CCP,RF, IgA,IgM,TNF-a,IgG,at the actiVe phase in RA patients were higher than those at inactiVe phase. The leVels of Serum TG and LDL were positiVely correlated with those of ESR,CRP,TNF-a,and serum HDL leVel was negatiVely correlated with that of ESR,CRP,TNF-a. ConcIusion：Lipid metabolism is remarkably abnormal in RA patients at actiVe phase and the blood lip-id,apart from ESR and CRP,may become an eValuation indeX for the actiVities of RA and one of the eValuation indeXes for the seVerity of rheumatoid arthritis as well.
