湖北省妇幼保健院近5年剖宫产率及剖宫产指征的变化分析

摘　　要：目的分析湖北省妇幼保健院近5年来剖宫产率及剖宫产指征的变化,探讨进一步降低剖宫产率的措施。方法回顾性分析2009年至2013年湖北省妇幼保健院分娩的孕产妇临床资料,比较了近5年来剖宫产率及剖宫产指征的变化。结果 1剖宫产率有显著下降趋势(P<0.01),2009年为59.36%,2013年为50.08%。2自2010年开始,剖宫产指征位于前6位的是:前次剖宫产、胎儿窘迫、社会因素、臀位(横位),巨大胎儿,羊水过少。3剖宫产指征中,前次剖宫产、羊水过少、臀位(横位)、双胎妊娠呈明显上升趋势,而胎儿窘迫及社会因素出现下降趋势。结论近年来剖宫产率已呈下降趋势,加强健康教育和孕期体重管理,提高超声诊断技术和阴道助产技术,提倡剖宫产后再次妊娠阴道分娩有望进一步降低剖宫产率。Objective To analyze the changes of cesarean delivery rates and cesarean section indications in recent five years in Maternal and Child Health Hospital of Hubei Province and to further explore the measures that can reduce the cesarean section rate.Methods Clinical data of women who gave birth in the Maternal and Child Health Hospital of Hubei Province from^2009 to 2013were retrospectively analyzed.The changes of cesarean delivery rates and cesarean section indications were examined.Results 1The rate of cesarean section was significantly decreased from 59.36%in 2009 to 50.08%in 2013（P〈0.01）.2The top six indications for cesarean section since 2010 were previous cesarean section,fetal distress,social factors,breech（transverse）,fetal macrosomia,oligohydramnios.3Analysis of cesarean section indications showed that women who had the history of cesarean section,oligohydramnios,breech（transverse）and twin pregnancy were more likely to receive cesarean section.The number of women who underwent cesarean section due to fetal distress and social factors tended to decrease.Conclusion The cesarean section rate tends to decrease in recent years.Strengthening the health education,controlling the weight of pregnant women,improving ultrasound diagnostic techniques and vaginal delivery technology,and advocating vaginal birth for women with prior cesarean section can help further reduce the rate of cesarean section.
