摘　　要：目的：对我院门诊患者退药情况进行分析，采取合理措施降低退药率，保证药品质量，从而提高我院药房管理质量，保证患者安全。方法：分析2013年6～12月我院门诊药房退药数据，根据退药科室、退药原因、退药剂型分类进行统计分析，并提出改进措施。结果：在退药原因中所占比例最大的是因医生开错处方导致退药的高达17.1%，患者退药金额共计52732.9元，其中神经类用药的退药金额最大，为23648.72元，所占比例为21.32%，皮肤外用药的退药比例和退药金额都较大，分别为48.34%和16705.85元，口服片剂占59.76%，注射剂占29.83%。结论：为加强医院管理，提高医务人员的责任心，强化医生合理用药意识，保证患者安全，减少退药，需要药师与医生协作与配合。Objective：Through our hospital outpatient drug withdrawal situation, analyze, take reasonable measures to reduce the incidence of drug withdrawal situation,to ensure the quality of drugs , thereby improving the quality of hospital pharmacy management to ensure patient safety.Methods：2013 6-12 months in our hospital pharmacy drug withdrawal data, according to the department drug withdrawal, drug withdrawal causes a retired pharmaceutical -type classification of statistical analysis, and proposed rationalization improvements. Results： The proportion of drug withdrawal biggest reason is because the doctor prescribing the wrong drug withdrawal resulted in up to 17.1%, for the departments case, the amount of drug withdrawal in patients with a total of 52,732.9 yuan, of which the amount of drug withdrawal biggest neurological drugs, was 23,648.72 yuan, the proportion was 21.32% , the proportion of drug withdrawal and drug withdrawal amount of skin external use of larger, respectively, 48.34% and 16,705.85 yuan, and for pharmaceutical dosage forms, oral tablets accounted for 59.76%, accounting for injections 29.83%.Conclusion：To enhance the quality of hospital management , physicians and other medical personnel to improve accountability, strengthen the doctor rational drug awareness, ensure patient safety, re duce the incidence of drug withdrawal situations drugs, pharmacists and doctors need collaboration and cooperation.
