小剂量酮咯酸氨丁三醇联合丙泊酚用于无痛胃镜下胃息肉电切术的临床研究

摘　　要：目的:评价小剂量酮咯酸氨丁三醇联合丙泊酚用于无痛胃镜下胃息肉电切术的安全性及有效性。方法:随机选择无痛胃镜下行胃息肉电切术患者80例。采取随机双盲法分为4组,每组20例。A组单纯丙泊酚麻醉组,B组丙泊酚复合芬太尼组,C组丙泊酚复合大剂量酮咯酸氨丁三醇组,D组丙泊酚复合小剂量酮咯酸氨丁三醇组。观察记录4组患者术中丙泊酚使用剂量、术中呼吸抑制情况、生命体征变动情况、手术后恢复清醒所需时间、术后疼痛评分、术后回访患者伤口出血发生率。结果:B、C、D组丙泊酚使用量低于A组(P<0.05);C、D组生命体征波动小(P<0.05);A、B组呼吸抑制发生率高于其他组(P<0.05);D组术后恢复清醒时间低于其他组(P<0.05);A组术后疼痛评分高于其他组(P<0.05);D组术后伤口出血发生率高于其他组(P<0.05)。结论:小剂量酮咯酸氨丁三醇联合丙泊酚用于无痛胃镜下胃息肉电切术安全有效。Objective：Evaluation of small dose of ketorolac ammonia butyl alcohol three combined with propofol for painless gastroscopy gastric polyp electrical safety and efficacy ofresection. Methods：Random selection of painless gastroscope gastric polyps high-frequency electrocoagulation resection in 80 cases. Take a random double blind method were divided into 4 groups, 20 patients in each group. Only in group A propofolgroup, B group of propofol and fentanyl group, C group of propofol combined with large dose of ketorolac tromethamine, Ding San alcohol group, D group of propofo] combined with small dose of ketorolac ammonia butyl alcohol threegroup. Observation of patients in the four groups of propofol dosage, intraoperative respiratory inhibition, vital signs changes, recovery after operationrecovery time, postoperative pain score, postoperative wound bleeding rate ofreturn. Results：B, C, D group of propofol consumption is lower than A group（P〈0.05）;C, D group of small fluctuation of vital signs（P〈0.05 ）;A, B group of respiratory inhibition ratehigher than other groups （P〈0.05）; D group, postoperative recovery time than the other group （P〈0.05）;A group postoperative pain score higher than the other group （P〈0.05）; the incidence is higher than the other group wound bleeding after operation in D group （P〈 0.05 ）. Conclusion： A small dose of ketorolac ammonia butyl alcohol three combined with propofolused in painless gastroscopy gastric polyps high-frequency electroblation safety.
