季节性接触性面部皮炎的变应原皮试及免疫疗效分析

摘　　要：目的：探究季节性接触性面部皮炎的变应原皮试及免疫疗效。方法对本院2009年12月~2011年12月皮肤科收治的87例面部皮炎患者的临床资料进行回顾性分析，选择本院皮肤科变态反应室制备实验变应原浸液为抗原，共有16种吸入物，于皮试前3 d，所有患者均停用皮质激素以及抗组胺药物，予以患者变应原皮试处理，观察患者的免疫效果。结果87例患者，65例患者均对同一种或者多种变应原呈阳性反应，其中阳性致敏变应原与发病季节吻合患者58例，予以患者免疫疗法后，33例治愈，27例显效，17例有效，10例无效，总显效率为68.96%，总有效率为88.50%。结论季节性面部皮炎主要致病因素是花粉抗原，于春、秋季节，面部皮炎发病率与季节致敏原阳性率相吻合，免疫疗法疗效显著，值得临床广泛推广与应用。Objective To explore the allergen skin test and immunotherapy efficacy of seasonal contact facial dermati-tis. Methods A retrospective analysis was conducted on the clinical data of 87 patients with facial dermatitis admitted and treated at the Department of Dermatology in our hospital from December 2009 to December 2011.Experimental al-lergen extract prepared by the allergic reaction lab of the Department of Dermatology was selected as the antigen,in-cluding 16 inhalants in total.All the patients stopped application of cortical hormone and antihistamine drug 3 d before the skin test.All the patients were prepared for the allergen skin test and the immunotherapy efficacy was observed. Re-sults In 87 patients,65 patients had positive reaction to one or multiple allergens.Positive allergen was consistent with the onset season among 58 patients.Immunotherapy resulted in 33 cases of cure,27 cases of significant therapeutic ef-fect,17 cases of therapeutic effect and 10 cases without therapeutic effect,with total significant effective rate of 68.96%and total effective rate of 88.50%. Conclusion Pollen antigen is the main pathogenic factor of seasonal facial dermatitis in spring and autumn.The incidence of facial dermatitis is consistent with the positive rate of seasonal allergen.Im-munotherapy has significant efficacy,which is worthy of widespread clinical application and promotion.
