摘 要:目的:探讨妊娠期高血压疾病孕妇发生脑血管疾病的关系及特点。方法:回顾性分析该院妇产科2010年9月-2014年1月收治的60例妊娠期高血压疾病患者的临床资料,根据病情分为重度组(22例)和轻度组(38例),比较两组患者妊娠期间和分娩后发生脑血管疾病及相关临床指标情况的差异。结果:重度组的发病孕周显著早于轻度组(P〈0.05);重度组出现呕吐、头痛、视力改变、抽搐的发生率均显著高于轻度组(P〈0.05)。重度组的收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、红细胞压积(HCT)、乳酸脱氢酶(LDH)、血肌酐(Cr)、血尿酸(UA)、24 h尿蛋白定量均显著高于轻度组,血红蛋白(Hb)显著低于对照组(P〈0.05)。重度组妊娠妇女脑血管疾病的发生率为45.45%,显著高于轻度组的7.89%(P〈0.05),妊娠期脑血管疾病与妊娠期高血压疾病严重程度呈显著的正相关(r=0.439,P=0.000)。结论:妊娠期高血压患者的疾病严重程度与脑血管疾病有显著的正相关关系,及时纠正妊娠期高血压对降低妊娠期脑血管病有积极意义。Objective: To explore the relationship between hypertensive disorder complicating pregnancy (HDCP) and cerebrovascular diseases in pregnant women and the clinical characteristics. Methods: The clinical data of 60 HDCP patients treated in gynecological and obstetrical department of the hospital from September 2010 to January 2014 were analyzed retrospectively, then the patients were divided into severe group (22 patients) and mild group (38 patients) , the incidence rates of cerebrovascular diseases and related clinical indexes during pregnancy and 'after delivery were compm'ed between the two groups. Results: The gestational week of onset in severe group was statistically significantly earlier than that in mild group ( P 〈 0. 05 ) ; the incidence rates of vomiting, headache, visual change and convulsions in severe group were statistically significantly higher than those in mild group (P 〈 0. 05 ) . Systolic blood pressure (SBP), diastolic blood pressure ( DBP), mean arterial pressure ( MAP), hematokrit ( HCT), lactic dehydrogenase ( LDH), serum creatinine ( Cr), blood uric acid (UA) levels and 24 -hour urine protein quantitation in severe group were statistically significantly higher than those in mild group, while hemoglobin level was statistically significantly lower than that in mild group ( P 〈 0. 05 ) . The incidence rate of cerebrovascuiar diseases in severe group was 45.45 % , which was statistically significantly higher than that in mild group (7.89%) (P 〈 0. 05 ) ; there was a significant correlation between cerebrovascular diseases during pregnancy and degree of severity of HDCP ( r = 0. 439, P = 0. 000 ) . Conclusion : there is a significant correlation between cerebrovascular diseases during pregnancy and the degree of severity of HDCP, timely correcting HDCP has active significance for reducing cerebrovascular diseases.
