摘 要:目的:比较钠钾镁钙葡萄糖注射液、5%葡萄糖注射液和9%氯化钠注射液对小儿围手术期血糖、血乳酸及血气电解质变化的影响。方法:选择ASAⅠ-Ⅱ级,年龄为1-5岁的择期手术患儿60例。手术时间选择不超过2 h且无较多失血者,随机分为乐加注射液组(A组)、5%葡萄糖注射液组(B组)和9%氯化钠注射液组(C组)。3组均采用静吸复合麻醉。分别于输液即刻(T0)与手术结束时(T1)测定患儿动脉血气、电解质、血糖和乳酸值。结果:B组输液后血糖显著升高,C组血糖显著下降(P〈0.05),但仍在正常值范围内,而A组输注前后则无明显变化(P〉0.05);C组血乳酸浓度值较其它两组显著下降(P〈0.05);B、C两组输液后Mg2+浓度降低(P〈0.05)。结论:钠钾镁钙葡萄糖注射液的电解质组成更符合人体生理浓度,能够在围手术期为小儿提供必需能量且对血乳酸、血糖及血气电解质水平影响小,是小儿安全、有效、理想的静脉补液选择。Objective: To compare the effects of sodium, potassium, magnesium, calcium and glucose injection, 5% glucose injection and 9% sodium chloride injection on blood glucose level, blood lactic acid level and change of electrolytes of blood gas analysis in children during perioperative period. Methods : Sixty ASA Ⅰ - Ⅱ 1 - 5 - year - old children undergoing selective operation were selected ; the operation time was not more than 2 hours and the children with massive hemorrhage were excluded; all the children were randomly divid- ed into group A (sodium, potassium, magnesium, calcium and glucose injection) , group B (5% glucose injection) and group C (9% sodi- um chloride injection) . Intravenous inhalational anesthesia was performed in the three groups. Artery blood gas, electrolytes, blood glucose and blood lactic acid levels were detected at the time of infusion (T0) and at the end of operation ( T1 ) . Results : After infusion, blood glucose level in group B increased significantly , while in group C, blood glucose level decreased significantly ( P 〈 0. 05 ) , but they were within notmal range, there was no statistically significant difference in blood glucose level between before and after infusion in group A ( P 〉 0. 05 ) ; lactic acid level in group C was statistically significantly lower than those in group A and group B ( P 〈 0. 05 ) ; after infusion, Mg2 + levels in group B and group C decreased significantly ( P 〈 0. 05 ) . Conclusion : The composition of electrolytes in sodium, potassium, magnesium, calcium and glucose injection is more agree with physical concentrations, this injection can provide necessary energy for children during perioperative period, and its impact on blood lactic acid level, blood glucose level and electrolytes of blood gas analysis is few, which is a safe, effective and ideal choice for intravenous infusion.
