摘  要:目的:以急性心肌梗死大鼠模型为研究对象,通过观察不同时间点四逆汤对该模型大鼠血浆中肌酸激酶(CK)、肿瘤坏死因子(TNF-α)活性的作用,探讨四逆汤对大鼠急性缺血性心肌梗死模型的CK和TNF-α动态药效影响。方法:将Wistar大鼠随机分为3组:心肌缺血组、假手术组、四逆汤组。心肌缺血组:左冠状动脉前降支起始部下2mm处结扎;假手术组:穿线不结扎;四逆汤组:预先给大鼠灌服四逆汤4天,第4天给药30分钟后,进行急性缺血性心肌梗死手术。按时间点0、5、10、20、40、60、90、120、150、180、210、360分钟采血,分别检测CK和TNF-α含量。结果:心肌缺血组、四逆汤组与假手术组比较,心肌缺血组的CK(10、20、60、90、120、150、360分钟)、TNF-α(120、150分钟),四逆汤组的CK(10、20分钟)含量明显升高,TNF-α含量无明显差异;四逆汤组与心肌缺血组比较,四逆汤组可显著降低血浆中CK(60、90、120、150、180、210、360分钟)、TNF-α(5、20、40、60、90、120、360分钟)的含量。假手术组、心肌缺血组CK含量与时间变化均为正相关关系,四逆汤组TNF-α含量与时间变化为负相关关系。结论:实验表明四逆汤能够有效地减少心肌酶的释放,改善缺血心肌的损伤,对大鼠缺血心肌具有积极明显的保护作用。各组间动态相关分析提示,通过降低炎性因子TNF-α含量,可能是四逆汤抗心肌缺血的重要机制之一。Objective: Through observing CK, TNF-α in plasma on rat models of acute myocardial infarction by Sini Decoction (SND), to explore dynamic efficacy on the rat acute myocardial infarction model with TNF-αand CK differential expression by Sini Decoction. Methods: Wistar rats were divided into three groups: myocardial ischemia group, sham-operation controlled group, and SND group. Myocardial ischemia rat groups were established by ligation of the left anterior descending coronary artery under 2mm from starting; sham-operation group rats were threaded but not ligation; SND group rats were given SND for 4days by intragastric gavage. The acute ischemic myocardial infarction operation is executed after received SND in the fourth day. According to the time of 0, 5, 10, 20, 40, 60, 90, 120, 50, 180, 210, 360min blood sampling, then detecting CK and TNF-α. Results: Comparing with sham-operation group, myocardial ischemia group and SND group, the contents of CK (10, 20, 60, 90, 120, 150min), TNF-α (120, 150min)in myocardialischemia group and the contents ofCK (10, 20min)in SND were significantly increased. Compared with the myocardial ischemia group, SND group could significantly reduce the con tents ofCK (60, 90, 120, 150, 180, 210, 360min), TNF-α (5, 20, 40, 60, 90, 120, 360min). It was a positive correlation between the content of CK and the time change in sham-operation group and myocardial ischemia group. It was a negative correlation between TNF-α and time changes. Conclusion: These evidence demonstrates that SND can reduce the release of myocardial enzymes, and improve the perfusion of myocardial ischemia, to protect positively to myocardial ischemia in rats. Prompts by correlation analysis in groups show that it's maybe one of the important mechanism with SND against myocardial ische mia by reducing the content of TNF-α as inflammatory factor.
