摘 要:目的了解兰州市各县区乡村医生的工作满意度,为进一步提高乡村医生工作效率,制定完善的管理措施提供一定的政策建议。方法 2012年7月选取兰州市各县区参加培训的552名乡村医生,在参考大量文献的基础上,结合兰州市各县区乡村医生的实际状况,编制符合乡村医生实际情况的有针对性的乡村医生满意度调查问卷。运用结构方程模型找出影响工作满意度的因素,提出改善建议。结果不同性别、从医年限乡村医生工作满意度得分比较,差异有统计学意义(P<0.05);不同受教育水平、职称、职业资格情况乡村医生工作满意度得分比较,差异均无统计学意义(P>0.05)。结构方程模型显示,业务素质与受教育水平的系数为7.075 6,与收入水平的系数为0.018 4;工作负荷与工作责任的系数为0.839 7,与工作任务的系数为0.681 7;工作环境与科室设备的系数为0.173 9,与绩效考核的系数为1.276 2;患者认可与患者信任的系数为0.764 2,与患者尊重的系数为0.713 2;工作满意与职业愉快感的系数为0.850 4,与工作自豪感的系数为0.718 5;离职倾向与离职念头的系数为0.718 7,与留职可能性的系数为0.805 6。结论兰州市各县区乡村医生的总体满意度较低,应该采取进一步措施提高乡村医生满意度,进而提高乡村医生的工作效率。Objective To investigate the village doctors' job satisfaction of each county area in Lanzhou city, in order to further improve the working efficiency of village doctors and to put forward some policy suggestion for making perfect management measures. Methods 552 village doctors, in every district Lanzhou, who took part in the training, were investigated using a doctor satisfaction questionnaire tally with the actual situation of the country doctors that was complied by referring to a large number of literature and review and combining with the actual condition of each county village doctor in Lanzhou city, using the structural equation model to find out the influencing factors of job satisfaction and put forward improving suggestions. Results The difference between sex and working years was significant ( P 〈 0. 05 ) . There were no difference among educational background, professional title, professional qualification ( P 〉 0.05 ) . Structural equation showed that the coefficient of professional quality and education level was 7.075 6, professional quality and income was 0. 018 4 ; the coefficient of work load and work responsibil- ity was 0. 839 7, work load and work task was 0. 681 7 ; The coefficient of work condition and department equipment was 0. 173 9, work condition and performance appraisal was 1. 276 2; the coefficient of patient approval and patient confidence was 0. 764 2 ,patient approval and patient respect was 0. 713 2; the coefficient of work satisfaction and work pleasure was 0. 850 4, work satisfaction and work pride was 0. 718 5 ; the coefficient of turnover intention and turnover thought was 0. 7187, turnover intention and employment stay possibility was 0. 805 6. Conclusion Village doctors' overall satisfaction is low in every district of Lanzhou city. It is necessary to take further measures to improve the village doctor satisfaction in order to enhance the working efficiency.
