促性腺激素释放激素类似物治疗不同年龄中枢性性早熟女孩的剩余生长能力评价

摘　　要：目的评价不同年龄开始使用促性腺激素释放激素类似物(GnRHa)治疗中枢性性早熟(CPP)女孩的剩余生长能力以及对最终身高的影响。方法将2002年3月—2003年5月来自温州医科大学附属育英儿童医院小儿内分泌科门诊的53例CPP女孩根据治疗起始时间分为低年龄组(A组)和高年龄组(B组),使用GnRHa对其治疗,在停止治疗后对全部患者进行每半年一次的长期随访,观察预测身高与实际终身高的差异。结果 (1)两组患者停止治疗时的剩余生长能力差异有统计学意义(P<0.05),A组患者在停止治疗后实际剩余生长百分比与预期剩余生长百分比相一致,而B组低于其预测剩余生长百分比(P<0.05),A组患者实际终身高明显高于B组患者(P<0.05)。(2)起始治疗时间与停药后患者的剩余生长能力呈负相关。结论低年龄开始GnRHa治疗预测其剩余生长能力较大,并且与实际预测较一致,而高年龄开始治疗的实际剩余生长往往低于预测,提示CPP患者宜尽早开展治疗。Objective To investigate the remaining growth ability and final body height in different aged girls with central precocious puberty（ CPP） after the treatment of gonadotropin- releasing hormone analog（ GnRHa）. Methods 53 girls with CPP selected from department of pediatric endocrinology of our hospital between March 2012 and May 2013 were divided into low age group（ group A） and high age group（ group B）. After treatment of GnRHa,all of them received long time follow up every half a year,we observing the difference between their predicted adult height and their final body height. Results（1）There was a significant difference between the two groups in remaining growth ability： group A showed no difference between final body height and predicted adult height（ P 〈 0. 05）; while in group B,the final body height was lower than the predicted adult height（ P 〈 0. 05）. Patients of group A were significant taller than those of group B when they turned into adults（ P 〈 0. 05）;（ 2） Patients’ remaining growth ability after treatment was negatively correlated to the then age of receiving treatment. Conclusion After treatment,girls with CPP starting treatment at low age are predicated higher remaining growth ability and it turns out that their final body height is similar to the target height. However,the high age group could not reach the predicted adult height. The results suggest that CPP patients should accept GnRHa treatment in earlier age.
