摘 要:目的:探讨小儿柯萨奇B组病毒感染的临床检测方法及患儿的血清学分型。方法:选取疑似小儿柯萨奇B组病毒感染的患儿651例作为研究对象,对患儿进行常规的血液生化检查、血液细菌学培养和类风湿因子检验,同时采用酶捕捉法(ELISA)分别进行柯萨奇B组病毒的Ig-M和Ig-G检测,并对柯萨奇病毒的血清分型进行鉴定。对检测结果呈阳性的患儿进行针对性的中西医治疗,观察治疗效果。结果:651例患儿中,经ELISA检测Ig-M抗体呈阳性患儿46例,阴性605例,阳性率为7.1%,特异性为92.9%;Ig-M抗体检测呈阳性患儿检测血清Ig-G抗体的滴度〉1∶16,确诊为柯萨奇B组病毒感染。经酶免法鉴别柯萨奇B组病毒血清分型为B1、B2、B3、B4、B5、B6型的分别有2、4、21、17、3、1例。经中西医结合用药治疗均痊愈出院。结论:柯萨奇B组病毒感染是常见的小儿病毒性感染疾病,1-3岁的儿童为易感人群,且感染具有散发性。采用ELISA检测Ig-M抗体具有灵敏度高、特异性好的特点,能够为患儿的柯萨奇B组病毒感染的诊断和血清学分型的鉴别提供准确有效的信息。Objective: To explore the clinical detection methods of Coxsackie B virus infection in children and serological typing. Methods: A total of 651 children suspected of Coxsackie B virus infection were selected as study objects, routine biochemical tests, blood bacterial culture and rheumatoid factor test were conducted; ELISA was used to detect Coxsackie B virus IgM and IgG; Coxsackie B virus se- rological types were identified. The children with positive test results were treated with corresponding Chinese and Western medicine treatment; ,the curative effects were observed. Results: Among 651 children, 46 children were found with positive IgM by ELISA, the positive rate was 7. 1% , the specificity was 92. 9% ; the titer of serum IgG in the children with positive IgM was higher than 1: 16, which was designed as the definite diagnostic criterion of Coxsackie B virus infection. The numbers of children with Coxsackie B virus serological type B1, B2, B3, B4, B5, B6 detected by ELISA were 2, 4, 21, 17, 3, 1 ; all the children were cured and left the hospital after Chinese and Western medicine treatment. Conclusion: Coxsackie B virus infection is a common viral infection in children, children aged 1 -3 years old are susceptible and the infection is sporadic. ELISA has the advantages of high sensitivity and specificity in detecting Ig - M, which can provide accurate and effective information for diagnosis and serological typing of Coxsackie B virus infection in children.
