腹腔镜手术治疗中期妊娠合并卵巢囊肿的效果观察

摘　　要：目的比较腹腔镜手术与传统手术治疗中期妊娠合并卵巢囊肿的效果,探讨腹腔镜手术的优势。方法选取2010年2月至2012年2月榆林市第一医院收治的中期妊娠合并卵巢囊肿患者75例进行腹腔镜辅助下卵巢囊肿剥除术,作为腹腔镜剥除术组;另外选取2008年1月至2010年1月榆林市第一医院收治的单胎中期妊娠合并卵巢囊肿患者75例给予传统开腹手术治疗,作为传统剖腹手术组。比较两组患者的手术成功率、术中出血量、手术时间、住院时间、并发症发生情况及妊娠结局。结果 1腹腔镜剥除术组患者的手术成功率(98.7%)显著高于传统剖腹手术组(84.0%)(χ2=10.1909,P<0.05);2腹腔镜剥除术组患者术中出血量、手术时间及住院时间均显著地少于传统剖腹手术组(t=168.67,98.600,63.722,P<0.05);3腹腔镜剥除术组术后并发症发生率显著地低于传统剖腹手术组(χ2=5.7920,P<0.05);4两组患者分娩结局比较差异无统计学意义(χ2=0.150,P=0.699)。结论腹腔镜手术治疗中期妊娠合并卵巢囊肿的效果优于开腹手术,其手术成功率高,出血量少、手术时间及住院时间短且并发症少,值得临床推广。Objective To compare the effect of lapamseopic surgery and traditional surgery in treating ovarian cyst during midtrimester pregnancy, and discuss the advantages of lapamscopic surgery. Methods A total of 75 cases of patients with ovarian cyst during midtrimoster pregnancy admitted in Yulin First Hospital from Feb. 2010 to Feb. 2012 were collected as group A, which were treated by laparoscopic surgery. Another 75 eases of patients with ovarian cyst during rnidtrimester admitted in Yulin First Hospital form Jan. 2008 to Jan. 2010 were collected as group B ,which were treated by traditional laparotomy. The surgery achievement ratios, amount of blood loss, operative time, length of stay, incidence of complications and pregnancy outcomes of the two groups were compared. Results ①The achievement ratio of group A was 98.7% , significantly higher than that of group B （ 84.0% ） （ X^2 = 10. 1909, P 〈 0.05 ） ;②Amount of bleeding, operation time, and hospitalization time of group A was significantly lower than group B （t = 168.67,98. 600,63. 722,P 〈0.05） ;③The incidence of complications of group A was significant lower than group B （ X^2 = 5. 7920, P 〈0. 05 ） ; （~）There was no significant statistical difference in full-term pregnancy rate between the two groups（ X^2 = 0. 150 ,P = 0. 699）. Conclusion Laparoscopic surgery in treating ovarian cyst during midtrimester has better effects than the traditional surgery. Laparoscopic surgery can improve the achievement ratio of operation, and reduce the amount of bleeding, shorten the operation time and hospitalization time, therefore, is worthy of clinical promotion.
