电子鼻咽喉镜下经咽鼓管口注药治疗分泌性中耳炎100例

摘　　要：目的探讨在电子鼻咽喉镜下经咽鼓管口注药治疗慢性分泌性中耳炎的疗效。方法回顾性分析100例（126耳）分泌性中耳炎患者,电子鼻咽喉镜下用直径1 mm的腰麻管从咽鼓管口插入,行咽鼓管吹张和疏通并在鼓室内注入a-糜蛋白酶和地塞米松,每3天1次,治疗3～4次。结果随访3～6个月后,痊愈82耳（82/126）,好转30耳（30/126）,无效14耳（14/126）,总有效率88.9%（112/126）。结论电子鼻咽喉镜下经咽鼓管口鼓室内注药治疗分泌性中耳炎直接、清晰、安全有效,操作简便,患者痛苦少。Objective To explore the treatment of secretory otitis media （ SOM） by earcatheter irrigation under electronic nasopharyngolaryngoscope .Methods 100 SOM cases （126 sides） were treated by earcatheter irrigation under electronic na-sopharyngolaryngoscope .After inserting a 1mm diameter tube used for lumbar hemp into the pharyngeal opening of auditory tube,the eustachian tube was dilated and drained before tympanic perfusion of α-chymotrypsin was conducted .The patients were treated once every 3 days and totally 3～4 times.Results 3 ～6 months after the treatment,86 ears were cured（86/126）.30 ears were improved （30/126） and 14 ears were unimproved（14/126）.The efficiency rate was 88.9%（112/126）. Conclusion Tympanic perfusion via eustachian tube under electronic nasopharyngolaryngoscope is a direct ,clear,safe,simple and efficacious method ,which may be reco mmended for the treatment of SOM .
