多民族高校社区高血压视网膜病变影响因素的临床分析

摘　　要：目的探讨多民族高学历高校社区高血压视网膜病变(hypertension retinopathy,HRP)的相关影响因素。方法对社区内管理的381例原发性高血压患者(essential hypertension,EH)进行眼底照相,结合患者的病程、血压、血糖、糖化血红蛋白、血脂等指标进行分析。结果在381例原发性高血压患者中,大学本科及以上学历297例(78%),少数民族259例(68%),汉族122例(33%),发生高血压眼底病变205例(53.81%)。发生高血压眼底病变患者与未出现高血压眼底病变患者比较在病程、收缩压、舒张压、是否伴发空腹血糖升高、低密度脂蛋白等方面有显著性差异(P<0.05),而在性别、脉压差、餐后2 h血糖、糖化血红蛋白、甘油三酯、胆固醇、高密度脂蛋白等方面无显著性差异(P>0.05)。结论高血压眼底病变的发生与高血压患者的病程长、血压控制不良、伴随血糖升高、血脂等指标控制不良相关,是今后社区工作防治的重点。Objective To analyses the related factors of the development of hypertension retinopathy（ HRP） in essential hypertension. Methods All 381 patients with essential hypertension in our community were evaluated with the method of the fundus photography and analysed retrospectively with the course of disease,blood pressure,level of glycosylated haemoglobin（ HbA1c）,fasting and postprandial blood sugar,blood lipid,and so on. Results Among the381 cases of essential hypertension,HRP were found in 205 cases（ 53. 81%）. Compared with the NHRP patients,the course of disease,the levels of blood pressure,fasting blood glucose and low density lipoprotein-cholesterol（ LDL-C） of the HRP patients were significantly different（ P 〈0. 05）. But there was no difference in the pulse pressure difference,the levels of glycosylated haemoglobin（ HbA1c）,postprandial blood sugar,Triglyceride（ TG）,cholesterol（ TC） and high density lipoprotein-cholesterol（ HDL-C）（ P 〉0.05）. Conclusion HRP morbidity is related to the long course of essential hypertension,abnormality levels of blood pressure,fasting blood glucose and LDL-C. So prophylactico-therapeutic measures of essential hypertension in community should be pay more attention.
