联合应用钬激光及封堵器在输尿管镜治疗输尿管上段结石的应用

摘　　要：目的比较联合应用钬激光及封堵器与单用钬激光在经输尿管镜治疗输尿管上段结石的疗效及安全性。方法输尿管上段单发结石患者108例分为两组:A组,68例,单用钬激光碎石;B组,40例,先置入封堵器套住结石,再用钬激光碎石。比较两组的手术时间、碎石成功率、输尿管穿孔、术后感染、术后出血率及术后输尿管狭窄率。结果 A组结石上移18例,B组结石上移3例。B组碎石成功率92.5%,高于A组的73.5%(P<0.05)。两组手术时间、输尿管穿孔率、输尿管狭窄率、术后感染率、术后严重出血率差异无统计学意义(P>0.05)。结论联合应用钬激光及封堵器治疗输尿管上段结石的疗效优于单用钬激光。Objective To compare the efficacy and safety of holmium laser combined with occlusion devices and holmium laser alone in ureteroscopic lithotripsy for the treatment of upper ureteral calculi. Methods The clinical data of 68 cases of single-use holmium laser（group A） and 40 cases of holmium laser combined with occlusion devices（group B） were retrospectively analyzed. The operation time,success rate of lithotripsy, and the rates of ureter perforation, postoperative infection and construction were compared between two groups. Results The success rate of lithotripsy was significantly higher in group B than that in group A（92.5% vs. 73.5%）（P^0. 05））. No significant differences were found in the operation time, the rates of ureter perforation, postoperative infection, hemorrhage and ureter stenosis between two groups（P^0. 05）. Conclusion Holmium laser combined with occlusion devices is more effective and safe than single-use holmium laser in the treatment of upperureteral calculi in ureteroscopic lithotripsy.
