穴位刺激调控对抑郁症患者心率变异及心电图特征的影响

摘　　要：目的 探讨穴位刺激调控对抑郁症患者心率变异及心电图特征的影响。方法 选择2012年1月~2013年12月在建德市第四人民医院住院的抑郁症患者98例,将其分为对照组（49例）及干预组（49例）,对照组给予百忧解治疗,干预组给予药物治疗的同时进行穴位刺激调控。干预3个疗程后,记录并比较两组的心率变异及心电图,记录症状自评量表（SCL-90）评分及不良反应。结果 干预组SCL-90评分中各项评分均低于对照组,差异均有统计学意义（P〈0.05）。干预组R-R间期总体标准差（SDNN）、平均R-R间期标准差（SDANN）、间期差值的均方根（RmSSD）、SDNN与SDANN超过50 ms的个数占总数的百分比（PNN50）等指标均高于对照组,低频频段（LF）/高频频段（HF）低于对照组,差异均有统计学意义（P〈0.05）。干预组在窦性心动过缓、窦性心律不齐、早期复极、室性早搏等发生比例上低于对照组,差异均有高度统计学意义（P〈0.01）。对照组不良反应发生率为10.2%（5/49）,干预组为2.0%（1/49）,差异有高度统计学意义（P〈0.01）。结论 依据中医辨证论治给予抑郁症患者穴位刺激调控联合药物治疗,可明显改善其24 h动态心电图特征及心率变异情况,且安全性较好,值得临床推广。Objective To study the effect of acupoint stimulation regulation for heart rate variation and electrocardio- gram （ECG） characteristics in patients with depression. Methods From January 2012 to December 2013, in the Fourth People＇s Hospital of Jiande City, 98 patients with depression were chosen and divided into the control group （49 cases） and the intervention group （49 cases）, the control group were given Prozac, the intervention group were given acupoint stimulation on the basic of drug treatment. After three course intetyention, heart rate variation ECG, SCL-90 and ad- verse reactions of two groups were recorded and analyzed. Resuits SCL-90 scores of the intervention group were lower than those of the control group with statistically significant differences （P 〈 0.05）. SDNN, SDANN, RmSSD, PNNS0 of the intervention group were higher than those of the control group, LF/HF was lower than that of the control group, the differences were statistically significant （P 〈 0.05）. In the intervention group, the early sinus bradycardia, sintis ar- rhythmia, bipolar, ventricular premature beat occurring rates were lower than those in the control group, the differences were statistically significant （P 〈 0.01）. The side effects, the incidence rate of the control group was 10.2% （5/49）, that of intervention group was 2.0% （1/49）, the differences were statistically significant （P 〈 O.O1）. Conclusion The im- provement effect of giving the corresponding acupuncture point stimulation control combination of drugs in 24 h ECG and heart rate variation for patients with depression is obvious on the basis of the medical treatment based on syndrome differentiation of traditional Chinese medicine and different individuals, and has good security, worth clinical promotion.
