摘　　要：目的 通过实施不合理医嘱事前干预,促进临床合理用药,保障患者用药安全、有效。方法 对2012年1月~2013年12月于福建医科大学附属宁德市医院,药师干预的837例不合理医嘱进行回顾性分析。结果 共审核出不合理用药869处,主要为医嘱输入错误、用法用量不合理、溶媒不合理、联合用药不合理、配伍禁忌和药物选择不合理等方面的问题,药师干预后,采纳率为90.33%。结论 不合理医嘱事前干预可有效减少不合理用药现象,应不断改进和完善事前干预的各项措施,以提高合理用药水平,保障医疗安全。Objective To implement intervention measures on irrational prescriptions so as to promote clinical rational drug use and ensure the safety and effectiveness of patients＇ medication. Methods A total of 837 irrational prescrip- tions intervened by pharmacists from the Hospital of Ningde City Affiliated to Fujian Medical University between Jan- uary 2012 and December 2013 were analyzed retrospectively. Results 869 cases of irrational drug use were examined, mainly including incorrect input of medical order, irrational usage and dosage of drug, improper solvent, unreasonable combined use,incompatibility and improper drug selection. After intervention by pharmacists, the proportion of adoption was 90.33%. Conclusion Intervention on irrational prescriptions beforehand can reduce irrational drug use effectively. Multiple intervention measures in advance aimed at irrational prescriptions should be improved and perfected constant- ly to improve rational drug use and ensure medication safety.
