反植皮法联合负压封闭引流技术治疗严重创伤并大面积皮肤撕脱伤的效果观察

摘　　要：目的 评价反植皮法联合负压封闭引流技术（VSD）治疗严重创伤并大面积皮肤撕脱伤的临床效果.方法 选取2008年1月～2013年5月本院收治的严重创伤并大面积皮肤撕脱伤患者49例,随机分为观察组和对照组,其中观察组25例采用反植皮法联合VSD进行治疗,对照组24例采用常规方法进行治疗,观察并比较两组的临床疗效、换药次数及住院时间.结果 观察组Ⅰ期愈合18例（72.0％）,Ⅱ期缝合、植皮以及皮瓣转移时间为（6.5±1.7）d,出院时创面愈合21例（84.0％）;对照组Ⅰ期愈合0例,Ⅱ期缝合、植皮以及皮瓣转移时间为（15.1±4.2）d,出院时创面愈合13例（54.2％）;两组比较,差异有统计学意义（P＜0.05）.观察组换药次数少于对照组,住院时间短于对照组,差异有统计学意义（P＜0.05）.结论 反植皮法联合VSD治疗严重创伤并大面积皮肤撕脱伤的临床效果较好,能较大程度地减少患者的换药次数,缩短住院时间,值得临床予以推广.Objective To evaluate the clinical efficacy of anti-graft law combined with vacuum sealing drainage （VSD） in the treatment of severe trauma and a large area of skin avulsion. Methods 49 cases of patients with severe trauma and a large area of skin avulsion treated in our hospital from January 2008 to May 2013 were selected and randomly divided into observation group （25 patients） and control group （24 patients）.The observation group were given with antigraft law combined with VSD,the control group were given with conventional methods of treatment.The clinical efficacy, dressing frequency and length of hospital stay of two groups were observed and compared. Results In observation group,18 cases （72.0%） were healed in period I ,period II suture,skin grafting and flap time was （6.5±1.7） d,21 cases （84.0%） were wound healed when leaving hospital;in control group,no cases were healed in period I ,period II suture, skin grafting and flap time was （15.1±4.2） d,13 cases （54.2%） were wound healed when leaving hospital;there were significant differences in the two groups （P〈0.05）.The frequency of dressing in observation group was less than that of control group,hospitalization time was shorter than that of control group,the difference was significant （P〈0.05）. Conclusion Anti-graft law combined with VSD in treatment of severe trauma and a large area of skin avulsion has significant clinical efficacy,can reduce the frequency of dressing,and shorten the hospitalization time,is worthy to be promoted in clinic.
