微创小切口跟腱松解结合Ilizarov牵拉技术治疗外伤性跟腱挛缩  

Treatment of Traumatic Cicatricial Contracture Achilles Tendon by Mini-Incision Achillotomy Combined with the Ilizarov Method

摘　　要：目的:探讨应用微创小切口跟腱松解结合Ilizarov牵拉技术治疗跟腱挛缩的临床疗效。方法:对2009年10月至2012年9月26例跟腱挛缩患者采用微创小切口跟腱松解结合Ilizarov牵拉技术进行治疗。结果:全部患者获得随访,平均随访时间达18个月,通过4-6个月牵拉矫形,患踝均矫形至或接近中立位,踝关节活动度从术前平均14°(10-20°)改善到38°(30-45°),患足行走负重满意。9例患者在牵拉过程中出现跖内侧皮肤疼痛,5例患者出现针眼无色渗液,无1例发生伤口或针眼感染、神经血管损伤或皮肤坏死等不良反应。结论:微创小切口跟腱松解结合Ilizarov牵拉技术是治疗跟腱挛缩的一种良好方法,值得推广。Objective：To investigate the clinical application of mini-incision achillotomy combined with the Ilizarov method in traumatic cicatricial contracture achilles tendon.Methods：From October2009 to September 2012,26 cases of traumatic cicatricial contracture achilles tendon were treated with the mini-incision achillotomy combined with the Ilizarov method,with 18 males and 8females.The mean age was 42years（range,17-56years）.Results：The mean follow-up time was18 months.After 4-6months＇ traction,all ankle joints was close to the neutral position and the ankle joint range of motion increased from a mean of 14°（range,10-20°）preoperatively to 38°（range,30-45°）at final follow-up.All cases achieved a better walking ability,and were satisfied with the result.Nine cases felt pain in plantar medial flaps and five cases showed exudates,but no infection,necrosis or wound rupture was found.Conclusion：The mini-incision achillotomy combined with the Ilizarov method showed good outcomes with a low rate of adverse reaction in the treatment of traumatic cicatricial contracture achilles tendon.
