摘 要:目的采用锥形束CT(cone-beam computed tomography,CBCT)与计算机辅助测量技术相结合,测量分析不同前牙不齐指数(little’s irregularity index,LII)骨性Ⅱ类错颌畸形下颌前牙区牙槽骨的厚度。方法选取66例骨性Ⅱ类错颌畸形患者的CBCT影像学资料,男性32例,女性34例。采用LII指数对下颌前牙进行轻、中、重度的不同拥挤度分类。使用Mimics 9.0测量分析软件测量下前牙牙根部位冠方、根中的唇舌侧的皮质骨厚度,松质骨厚度及骨总厚度;根尖处唇舌侧的皮质骨厚度、松质骨厚度和骨总厚度及根尖处总骨厚度、总松质骨厚度。结果在根尖处总松质骨厚度和根尖处舌侧松质骨厚度男性>女性(P<0.05);重度拥挤的冠方唇侧骨总厚度、冠方唇侧皮质骨厚度>中度、轻度拥挤者(P<0.05);重度拥挤的冠方唇侧松质骨厚度>中度拥挤者(P<0.05)。男性、女性的支持骨量和拥挤度间具有一定相关性(|r|>0.3)。结论下颌前牙区牙槽厚度与下前牙的拥挤程度存在一定相关性;除了根尖处总松质骨厚度和根尖处舌侧松质骨厚度外,下颌前牙的支持骨测量无性别差异。Objective To measure alveolar bone thickness in mandibular anterior region of class Ⅱmalocclusion on cone-beam computed tomography(CBCT) sections. Methods The CBCT data of 66 cases of class Ⅱ malocclusion(male 32 and female 34) were selected for this study. An irregularity index was used to categorize these data as having mild,moderate,or severe crowding. Then,the following parameters were measured on the sections corresponding to root region of mandibular incisors with the Mimics 9. 0 software:labial,lingual,and total alveolar thickness of cortical and cancellous bone on the region of alveolar bone crest and middle root region; labial,lingual,and total alveolar thickness of cortical and cancellous bone on the root apical region,and total cancellous bone thickness and distance between the vestibular and lingual cortices.Results The total and lingual cancellous bone thickness of the root apical region were greater in the males than in the females(P〈 0. 05). The total,labial cortical and labial cancellous bone thickness of the alveolar bone crest region of the severe crowding cases were greater than those of the mild and moderate crowding cases(P〈 0.05). The total and labial cortical thickness of the alveolar bone crest region of the severe crowding cases were greater than those of the moderate crowding cases(P〈 0. 05). Additionally,by using Pearson correlation analysis,significant correlations were determined between incisor crowding and alveolar bone thickness in male and female( | r | 〉0. 3). Conclusion Significant correlation is found between the degrees of mandibular incisor crowding and alveolar bone dimensions. Except for the total and lingual cancellous bone thickness of the root apical region,there is no gender differences in basal bone dimensions.
