自体输血在冠状动脉搭桥手术中的应用

摘　　要：目的探讨冠状动脉搭桥患者应用自体采血的可行性。方法选择2012年1-6月期间做冠状动脉搭桥手术患者,对符合自体采血的患者进行自体采血,做为自体输血组,同时随机选取100例年龄、性别均匹配的未进行自体输血的患者,做为异体输血组,对两组患者术前、术后及出院前测定血液学参数,应用SPSS13软件进行统计分析。结果自体输血组采血前血红蛋白（HB）、红细胞比容（HCT）和红细胞数（RBC）,与术前、术后第1天以及出院前指标的差异有统计学意义（P0.05）。结论对于择期符合自体输血条件的冠状动脉搭桥手术患者,可以进行自体输血。既保证手术顺利进行,又可避免异体血输注中输血反应以及疾病传播。Objective To evaluate the feasibility of pre stored autologous blood transfusion in coronary artery bypass grafting. Methods Blood was collected from the patients who were in accordance with the criteria of autologous blood transfusion from 1 January to 30 June 2012. One hundred cases without autologous blood collection were set up as control. Hematology parameters were statistically analyzed with SPSS13.0 software in both groups of the patients of pre-and post operation and before discharge. Results In the group of pre-stored autologous blood transfusion, the parameters of hemoglubin（HB）, hematocrit （HCT）,and red blood cells （RBCs） had a significant difference before blood collection com- pared with those of pre-operation,the first day of post-operation, and just before discharge （P〈0.05）. No statistically significant difference of HB,HCT, and RBCs was noted in pre- and post-operation, and before discharge between the two groups of autologous and homologous blood transfusion （P〉0. 05）. Conclusions Pre-stored autologous blood transfusion may be used in the patients with right elective coronary artery bypass grafting surgery, from which the patients may benefit and be protected from adverse transfusion reaction and infection with infectious diseases.
