摘  要:目的:探讨宫腔镜术前肛门内放置卡孕栓扩张宫颈的临床效果。方法:选取2011年6月-2012年6月在本院行宫腔镜手术且无禁忌证的200例患者,按照随机数字表法将其分为卡孕栓组(A组)和米索前列醇组(B组)各100例,两组均于术前2 h给药,观察比较两组患者的宫颈松弛情况、手术时间及手术出血量。结果:两组宫颈软化均达到较好效果,但比较差异无统计学意义(P>0.05)。A组子宫内膜息肉、子宫黏膜下肌瘤的手术时间均明显短于B组,手术出血量均明显少于B组,差异均有统计学意义(P<0.01)。A组恶心、呕吐发生率、腹痛发生率及发热发生率均明显低于B组,差异均有统计学意义(P<0.05)。结论:卡孕栓与米索前列醇均能有效地扩张宫颈,而卡孕栓能够减少出血量,缩短手术时间,减少患者的副反应,值得临床推广使用。Objective:To investigate the clinical effect of applying carboprost methylate to dilation of the cervix via anus placement prior in hysteroscopic surgery.Method:200 patients underwent hysteroscopic surgery and no contraindication in our hospital from June 2011 to June 2012 were selected,they were divided into the carboprost suppository group and the misoprostol group according to the random number table method,100 cases in each group.The two groups were respectively managed with the previous drug before surgery.The relaxation degree of the cervix,operative time and intraoperative blood loss of two groups were observed and compared.Result:The cervix softening achieved good effect in both groups,but there was no statistically significant difference between two groups(P〉0.05).The operative time of endometrial polyps,submucous myoma of uterus in group A were significantly shorter than group B,intraoperative blood loss of group A was significantly less than group B,the differences were statistically significant(P〈0.01).The incidence of nausea,vomiting,abdominal pain and fever in group A were significantly lower than those of group B,the differences were statistically significant(P〈0.05).Conclusion:Carboprost methylate and misoprostol can effectively expand the cervix,yet the former can reduce blood loss,shorten the operative time,reduce the patient's adverse reactions,worthy of clinical use.
