阴道彩超对早孕期胎盘绒毛植入药物治疗疗效的评估

摘　　要：目的探讨经阴道彩色多普勒超声对评估早孕期胎盘绒毛植入不全流产的药物治疗效果的价值。方法对59例流产后阴道反复出血并已接受甲氨喋呤及米非司酮等药物治疗的患者进行超声检查追踪,分别采用经腹部二维超声及经阴道彩色多普勒监测病变的范围、大小、回声及血流等参数的改变。结果经腹部二维超声观察,可发现其中32例病灶大小、回声等明显变化,余下27例多次复查变化不明显,而经阴道彩色多普勒超声不仅分辨力提高,较清晰地显示病灶,更能通过显示病灶部位血流变化,评估病变经治疗后的改变,从而正确评估疗效。结论经阴道彩色多普勒超声检查对早孕期胎盘绒毛植入不全流产的疗效评估有较大的临床意义,且优于经腹部二维超声检查。Objective To explore the value of transvaginal color doppler flow imaging in drug treatment of early pregnancy placental villi implantation with incomplete abortion. Methods 59 cases of patients with abortion after repeated vaginal bleeding and treated with methotrexate and mifepristone as well as other drug treatment were given ultrasound track, and the abdominal two - dimensional ultrasound and TVCD were used to monitor the change of parameters such as lesions, size, echo and blood flow. Results The abdominal two - dimensional ultrasound showed obvious changes of 32 eases of le- sions, echo, etc. , while the remaining 27 cases had no obvious changes after repeated review. However, TVCD not only im- proved the resolution and clearly showed the lesions, but also evaluated the pathological changes after treatment through the demonstration of blood flow changes in lesions, therefore evaluated the curative effect accurately. Conclusion TVCD has great clinical significance for early pregnancy placenta villi implantation with incomplete abortion in the evaluation of curative effect, and is better than the abdominal two - dimensional ultrasound.
