足底反射区按摩联合股动脉注射药物治疗0级糖尿病足效果研究

摘　　要：目的观察足底反射区按摩联合股动脉注射药物对0级糖尿病足的临床疗效。方法将96例符合Wagner分级标准的0级糖尿病足患者随机分为对照组、治疗1组、治疗2组3组,各32例。对照组给予常规控制血糖,静脉滴注活血化瘀、扩张血管、营养神经药物治疗;治疗1组除上述治疗外加足底反射区按摩;治疗2组在治疗1组基础上股动脉注射利多卡因加罂粟碱。3组均10 d为1个疗程,2个疗程后统计疗效,观察治疗前后踝肱指数(ABI)、腓总神经运动传导速度(MNCV)、感觉传导速度(SNCV)。结果治疗2组与对照组、治疗1组总有效率比较差异均有统计学意义(P<0.05),治疗2组疗效优于对照组、治疗1组;ABI测定显示治疗2组优于对照组、治疗1组,差异均有统计学意义(P<0.05);治疗后MNCV、SNCV,治疗1组、治疗2组与本组治疗前比较差异有统计学意义(P<0.05),改善明显;治疗2组治疗后MNCV、SNCV与治疗1组治疗后差异具有统计学意义(P<0.05),改善优于治疗1组。结论足底反射区按摩联合股动脉注射药物能改善0级糖尿病足患者肢体麻木、刺痛、间歇性跛行、静息痛等症状,能够有效预防糖尿病足部溃疡。Objective To observe the clinical effect of plantar reflex zone massage combined with femoral artery injection on 0 grade diabetic foot. Methods 96 patients with Wagner classification standard grade 0 diabetes were randomly divided into group A, B and C, 32 cases in each group. The group A was given conventional glucose control, venous blood circulation, expansion of blood vessels, nerve nutrition therapy; group B, in addition to the above treatment, plus foot reflex zone massage;Group C femoral artery in group B based on the injection of lidocaine plus pa- paverine. The 3 groups were 10 d for 1 course ,2 courses later the statistical effect and peroneal nerve motor conduction velocity （ MNCV）, sensory conduction velocity （SNCV）. Results There were statistically significant differences he- tween group C and group A, group B on the overall efficiency （ P 〈 O. 05 ）, curative effect of group C is better than that of group A and group B. MNCV ,SNCV in group B and group C after treatment were improved as compared with those before treatment,the difference was statistically significant （P 〈 0.05 ）. The improvement of MNCV and SNCV after treatment in group C was superior to that of group B. Conclusion The plantar reflex zone massage combined with femoral artery injection can improve 0 grade diabetic foot patients limb numbness, tingling, intermittent claudication, rest pain and other symptoms, to be effective in the prevention of diabetic foot ulcer.
