膝骨关节炎患者关节镜清理前后关节液中MMP1、MMP3、IL-1的变化及意义

摘　　要：目的：研究膝骨关节炎患者关节镜清理前后关节液中基质金属蛋白酶1、3（MMP1、MMP3）及白介素-1（IL-1）含量的变化及其临床价值。方法：将2011年5月-2013年12月间我院骨科收治的膝关节骨性关节炎患者60例纳入研究,进行关节镜清理手术,比较手术前和手术后3、6、12个月时的膝关节活动功能以及关节液中MMP1、MMP3、IL-1含量。结果：手术后3、6、12个月时,患者关节液中MMP1、MMP3、IL-1的mRNA和蛋白含量均低于手术前,伸膝最大角度和屈膝最大角度均高于手术前（P均〈0.05）;关节液中MMP1、MMP3、IL-1的含量与伸膝最大角度和屈膝最大角度呈负相关。结论：关节镜清理手术能够减少关节液中MMP1、MMP3、IL-1的含量,同时改善膝关节活动功能,是治疗膝骨关节炎的理想方法。Objective：To study the changes of MMP1,MMP3 and IL-1content in synovial fluid of knee osteoarthritis patients before and after arthroscopic debridement.Methods：A total of 60 cases of knee osteoarthritis admitted from 2011 May to 2013 December were enrolled and treated with arthroscopic debridement.Before surgery and 3,6,12 months after surgery,knee joint function and MMP1,MMP3 and IL-1content in synovial fluid were compared.Results：3,6,12 months after surgery,MMP1,MMP3 and IL-1content in synovial fluid were lower than those before surgery;knee maximum extension angle and maximum flexion angle were higher than those before surgery;MMP1,MMP3 and IL-1content in synovial fluid were negatively correlated with knee maximum extension angle and maximum flexion angle.Conclusion：Arthroscopic debridement surgery can reduce MMP1,MMP3 and IL-1content in synovial fluid and improve knee function.
