阴道B超介入下治疗卵泡未破裂黄素化及卵巢巧克力囊肿临床观察

摘　　要：目的:探讨阴道B超介入下治疗卵泡未破裂黄素化及卵巢巧克力囊肿临床疗效。方法:将80例卵泡未破裂黄素化及卵巢巧克力囊肿患者随机分为对照组与观察组,每组患者数均为40例。观察组行经阴道B超介入治疗,对照组采用卵巢巧克力囊肿剥除术。比较两组临床疗效、破卵率、妊娠率、囊肿消失率、囊肿复发率及痛经缓解率、治疗前后血清卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E2)水平变化、不良反应发生率。结果:观察组临床治疗总有效率高于对照组,差异具有统计学意义(P<0.05);观察组破卵率、妊娠率、囊肿消失率、痛经缓解率均高于对照组,且囊肿复发率明显低于对照组,差异具有统计学意义(P<0.05);观察组治疗后6个月上述指标水平与治疗前相比,差异均具有统计学意义(P<0.05),但随后逐渐恢复至治疗前水平;观察组术后不良反应发生率稍低于对照组,差异无统计学意义(P>0.05)。结论:经阴道B超介入下治疗卵泡未破裂黄素化及卵巢巧克力囊肿临床疗效明显,虽对卵巢储备功能存在一定的影响,但术后联合GnRH-a治疗,患者均可恢复。Objective：To evaluate the clinical efficacy of interventional treatment of vaginal B ultrasound on unruptured follicles and ovarian chocolate cysts.Methods：A total of 80 patients with unruptured follicles and ovarian chocolate cysts were randomly divided into control group and observation group in accordance with the law of parity figures,with 40 cases in each group.The observation group wastreated with transvaginal B-intervention therapy,and the control group underwent cystectomy of ovavian chocolate cysts.The clinical efficacy,broken egg rate,pregnancy rate,the rate of disappearance of cysts,cyst recurrence rate and dysmenorrhea remission rate,serum FSH,LH,E2 levels before and after treatment,incidence of adverse reactions were compared.Results：The total clinical effective rate in observation group was 95.00%,significantly higher than that of control group（75.00%）（P〈0.05）;broken egg rate,pregnancy rate,the rate of disappearance of cysts,dysmenorrhea remission rate of the observation group was significantly higher（P〈 0.05）,but cyst recurrence rate was significantly lower than the control group（P〈0.05）.6months after treatment,the detected indexes levels were significantly different from that before treatment（P〈0.05）.Incidence of postoperative adverse reactions of the observation group was slightly lower than that of the control group.Conclusion：B-intervention therapy is effective on vaginal unruptured follicles and ovarian cysts,although it affects ovarian reservation function,postoperative GnRH-a treatment can help to recover the function.
