摘　　要：目的:探讨左卡尼汀对维持性血液透析(MHD)微炎症和营养状况的作用机制。方法:将收治的维持性血液透析大于6个月的患者共44例分为治疗组和对照组,治疗组在每次结束透析时静脉注射左卡尼汀注射液1.0g,随访3个月。监测治疗前,治疗后1个月及3个月血红蛋白(Hb)、血清白蛋白(Alb)、前清蛋白(PA)、转铁蛋白(TF)、超敏C反应蛋白(hs-CRP)、白细胞介素(IL-6)IL-6、肿瘤坏死因子-α(TNF-α)等指标。结果:治疗1及3个月后,治疗组Alb、PA以及Hb与对照组及治疗前比较上升明显;而TF及炎性指标hs-CRP、IL-6、TNF-α值则逐渐下降,治疗1和3个月后,与治疗前及对照组相比,差异有统计学意义(P<0.05)。结论:左卡尼汀是改善MHD患者微炎症及影响状态的安全有效的手段。Objective： To investigate the effects of Levocarnitine on nutritional and microinflammation status of patients with maintenance hemodialysis （MHD）. Methods; A total of 44 patients who undergoing maintenance hemodialysis for more than six months were divided into the treatment group and the control group with 22 eases in each group. Patients in the treat- ment group received intravenous injection of L-Garnitine 1.0 g every time at the end of the hemodialysis treatment. The follow- up study was conducted for 3 months. Blood biochemical indicators, serum Hb, Alb, PA, TF, hs-CRP, IL-6, and TNF-a were measured and compared before the experiment, 1 month and 3 months after the treatment. Results： The Alb, PA, Hb level were significantly increased in the treatment group 1 and 3 months after the treatment. But TF, hs-CRP, IL-6, TNF- a level were significantly decreased, and there were significant differences in these levels between 1 and 3 months after treatment, also between the control group and observation group （P〈0.05）. Conclusion： L-Carnitine can improve the nutritional and microinflammation status of patients with maintenance hemodialysis.
