摘　　要：目的:探讨定期尿激酶封管对血液透析患者长期颈内静脉留置导管的影响。方法:选取于本院行颈内静脉留置双腔导管进行血液透析治疗的患者62例,随机分为观察组与对照组,观察组采用25000U/mL尿激酶+25mg/mL肝素溶液24h停留封管,对照组采用5mg/mL肝素生理盐水封管,观察两组凝血功能、导管留置时间、实际血流量、导管通畅率等指标。结果:两组均行28个月观察,观察组导管留置时间、实际血流量及导管通畅率均明显高于对照组,差异有统计学意义(P<0.05);两组凝血功能比较,差异无统计学差异(P>0.05)。结论:25 000U/mL尿激酶+25mg/mL肝素溶液24h停留封管可有效预防导管血栓形成,提高置管时间,增加透析治疗血流量,改善导管功能作用明显,临床应广泛推广应用。Objective.. To explore the effect of regular urokinase sealing on long-term internal jugular vein indwelling catheter of hemodialysis patient. Methods： A total of 62 patients who underwent hemodialysis by indwelling double channel catheter in the internal jugular vein in our hospital were included and were randomly divided into observation group and control group. Patients in the observation group were given 25000U/mL urokinase and 25mg/mL heparin solution which were indwelled in the catheter for 24h; while 5 mg/mL heparin normal saline was used to seal the catheter in the control group. The indicators such as coagulation function, catheter indwelling time, actual blood flow volume, patency rate of the catheter of the two groups were observed. Results： Follow-up were conducted on both groups for 28 months. The indwelling time, actual blood flow volume, and the patency rate in the observation were all significantly higher than those in the control group （P〈 0.05）. The difference of the coagulation function between the two groups was not statistically significant （P〉0.05）. Oonclusions： It can be concluded that 25 000U/mL urokinase and 25 mg/mL heparin solution indwelling in the catheter for 24h can effectively prevent occurrence of thrombosis, prolong indwelling time, increase blood flow volume and improve the catheter function, thus the hemodialysis patient＇s survival quality is enhanced and it can be widely used in the clinics.
