中老年股骨颈骨折全髋关节置换术和骨折内固定术临床比较

摘　　要：目的:探讨中老年股骨颈骨折全髋关节置换术及骨折内固定手术的临床疗效。方法:置换组患者给予全髋关节置换手术治疗,内固定组患者给予骨折内固定手术治疗,记录两组患者术中出血量、手术时间、术后至下床活动时间、预后效果、住院时间、并发症发生情况、治疗费用等,给予统计学分析后得出结论。结果:置换组患者手术时间、术中出血量、术后至下床活动时间、住院时间均显著少于内固定组,但其手术费较高;置换组术后并发症发生率仅为11.76%,显著低于内固定组的31.58%;两组患者近期疗效均较为满意,但置换组远期疗效显著优于内固定组(P<0.05)。结论:临床医生应根据股骨颈骨折患者实际情况,如病情、骨质疏松程度、经济情况等进行综合考虑,选择合适的治疗方案,从而有效提高患者临床疗效及预后,保障其生活质量。Objective：To investigate the clinical effect of fixed operation of total hip joint replacement and internal fracture of femoral neck fractures in the elderly.Method：The permutation group was treated with total hip replacement operation,the internal fixation group was treated with fixed operation.The bleeding volume,operation time,postoperative ambulation,and prognosis,hospitalization time,complications,treatment costs of two group were recorded and compared.Result：The operation time,intraoperative bleeding volume,posto-perative to ambulation time,hospitalization time of the permutation group were significantly less than that of the internal fixation group,but its operation cost was high.The incidence rate of the permutation group was 11.76%,was significantly lower than that of the internal fixation group.The short future curative effect of two groups were satisfactory,but long term effect of the replacement group was significantly better than that of the internal fixation group（P〈0.05）.Conclusion：Clinician should be based on the actual situation of patients with femoral neck fracture,considering such as disease,osteoporosis,economic situation,choose the appropriate treatment,so as to improve the therapeutic effect and the prognosis of patients,guarantee the quality of life.
