摘 要:目的分析干预前后抗菌药物临床应用合理性及干预措施的效果,为合理应用抗菌药物提供理论依据。方法分别抽取2011、2012年住院患者病历各2 000份作为非干预组及干预组,通过干预措施,对抗菌药物使用进行对照研究,应用SPSS13.0软件对所有数据进行统计分析。结果非干预组抗菌药物使用强度、人均抗菌药物费用、抗菌药物使用率分别为23.21±6.10、(1057.25±379.12)元、42.35%,干预组分别为13.59±5.62、(572.43±150.35)元、31.20%,干预组显著低于非干预组,差异有统计学意义(P<0.01);干预组病原学送检率为47.25%,显著高于非干预组29.80%,差异有统计学意义(P<0.01);非干预组围手术期预防用抗菌药物品种选择、用法用量、使用疗程及给药时机合理率分别为68.27%、71.22%、42.85%及68.74%,干预组分别为90.16%、87.57%、80.40%及89.35%,干预组抗菌药物使用合理率显著高于非干预组,差异有统计学意义(P<0.01);干预组围手术期联合用药比例为5.99%,低于非干预组8.16%,差异有统计学意义(P<0.05)。结论实施干预措施可以显著减少抗菌药物不合理应用,提高了医务人员抗菌药物合理使用的观念,干预措施具有可行性及有效性。OBJECTIVE To evaluate the role of the intervention with the rational application of antimicrobial agents so as to provide theoretical basis for rational use of antimicrobial agents.METHODS The respective 2000 inpatient medical records of 2011 and 2012were selected as the intervened group and the non-intervened group.The main indexes were compared by case control study,and the statistical analysis was performed by means of SPSS13.0software.RESULTS The intensity of use,the per capita cost and the usage rate of antibacterial agents of the nonintervened group were 23.21±6.10,1057.25±379.12 yuan and 42.35%,respectively,and were 13.59±5.62,572.43±150.35 yuan and 31.20%,respectively in the intervened group which were significantly lower than the non-intervened group(P〈0.01).The etiologic sample-delivery rate in the intervened group was 47.25%,which was significantly higher than 29.80%in the non-intervened group(P〈0.01).In the non-intervened group,the reasonable rates for selection of preventive antibacterials during the perioperative period,usage and dosage,treatment course,delivery time were 68.27%,71.22%,42.85% and 68.74%,respectively.Those in the intervened group were 90.16%,87.57%,80.40% and 89.35%,respectively,which were significantly higher than the non-intervened group,(P〈0.01).The combined medication ratio during the perioperative period in the intervened group was 5.99%,which was significantly lower than 8.16%in the non-intervened group(P〈0.05).CONCLUSIONS Intervention can not only decrease the unreasonable application of antibacterial agents,but also enhance the concept of rational use of antibacterials in medical personnel.The intervention measures are feasible and available.
