Ⅰ类切口手术抗菌药物应用调查分析

摘　　要：目的调查《抗菌药物临床应用专项整治活动方案》实施后,医院Ⅰ类切口手术抗菌药物应用合理性的变化趋势,为医院抗菌药物的合理应用提供参考。方法采用回顾性调查方法,随机抽取医院2011年1-4月及2013年1-4月Ⅰ类切口手术病例,对比分析Ⅰ类手术抗菌药物预防性应用的各类指标,并进行评价。结果医院2013年1-4月较2011年同期Ⅰ类切口手术抗菌药物使用率由90.86%下降至21.91%;选用品种由10种下降至3种;2011年的186例Ⅰ类切口手术患者使用抗菌药物用在术前30min^2h的135例,占72.58%;2013年同期的178例首剂用在术前30min^2h的176例,占98.87%;用药持续时间<48h的比率由14.20%提高至87.20%;合理使用率由41.94%上升至94.94%。结论 2013年医院Ⅰ类切口手术抗菌药物应用的合理性有明显提高,说明法律法规的实施能有效地改善抗菌药物不合理应用。OBJECTIVE To investigate and analyze the tendency of rationality of use of antibacterial drugs for typeⅠ incision surgery before and after the implementation of the special rectification activitiy program of the antibiotics clinical application in the hospital so as to provide clinical reference for rational use of the drugs.METHODS The data of the utilized antibacterial in typeⅠ incision surgery were retrospectively analyzed during Jan-Apr of 2011 and the same time of 2013 in the hospital,and a comment was made on rationality of the utilization.RESULTS The utilization rate of antibacterial drug in typeⅠ incision surgery decreased from 90.86%in 2011 to 21.91%in 2013;the selection of varieties reduced from 10 to 3;the preoperative used in 0.5-2hours of the first agent increased from 72.58%（135cases,186 cases in total）in 2011 to 98.87%（176cases,178 cases in total）in 2013;the ratio of drug duration less than 48 hours rose from 14.20%to 87.20%,and the reasonable utilization rate of antibacterial drug increased from 41.9% to 94.9%.CONCLUSION The rationality of use of antibacterial drugs is significantly improved after implementation of the special rectification activities in the hospital,which illustrates that the unreasonable use of antibacterial drugs can be effectively improved through the implementation of the regulations.
