骨折内固定术后早期感染保留内固定物治疗效果分析

摘　　要：目的探讨股骨开放性骨折内固定（OFFIF）术后早期感染保留内固定物治疗效果。方法选择2008年6月-2011年10月股骨开放性骨折行OFFIF术后发生早期感染的患者24例,在未改变内固定的前提下进行清创、冲洗引流及抗感染治疗。结果 16例患者拔管后伤口愈合出院,4例患者行二次清创、冲洗引流后伤口愈合出院,2例患者行3次清创、冲洗引流后伤口愈合出院,仅有2例患者伤口未能及时愈合,在积极换药和中药口服1个月后伤口愈合;随访5个月～3年,平均10.5个月;24例患者中,20例伤口完全愈合未复发,愈合率为83.3%。结论 OFFIF术后早期感染在保留内固定物的前提下,进行清创、冲洗引流及抗感染治疗效果良好。OBJECTIVE To investigate the effect of internal fixation reservation for infection treatment after internal fixation for open femur fracture（OFFIF）.METHODS Totally 24 patients who had postoperative infections after OFFIF from Jun.2008 to Oct.2011 in our hospital were collected.Debridement,irrigation and drainage and antiinfection treatment were performed under the premise of no changes in the fixation.RESULTS Totally 16 patients had wound healed and discharged after extubation,4patients underwent secondary debridement,irrigation and drainage and were discharged after wound healed,two patients underwent three debridement,irrigation and drainage and were discharged after wound healed,only 2patients did not had their wounds healed timely and their wounds were healed after one month of oral administration of traditional Chinese medicine and positive dressing.The follow-ups continued for 5months to 3years,with an average of 10.5months.Totally 20 patients had wound completely healed and no recurrence（the healing rate 83.3%）.CONCLUSION Under the premise of internal fixation reservation,postoperative early infection after OFFIF,debridement,irrigation and drainage and antiinfection treatment can achieve good results.
