摘　　要：目的：探讨肥胖男性瘦素（Leptin）水平与男性不育相关各精液参数以及内分泌激素的相关性。方法从上海交通大学医学院附属仁济医院男科门诊就诊患者中选取30例，设为肥胖组；另选取正常健康男性10例为对照组。对两组都进行精液参数分析、内分泌激素检测和血清Leptin水平检测。结果对照组精子密度明显高于肥胖组（P＜0.01），对照组A级活动精子和B级活动精子百分比都明显高于肥胖组（P＜0.01）。对照组促卵泡生成素（FSH）、促黄体激素（LH）和睾酮（T）水平明显高于肥胖组（P＜0.05）。对照组血清Leptin水平明显低于肥胖组（P＜0.01）。血清Leptin水平与精子密度之间有显著负相关性（P＜0.05）血清Leptin水平与FSH、LH、T水平之间均有显著负相关性（P＜0.05）。结论血清Leptin水平与精液各项参数以及内分泌激素有非常密切的关系，可能抑制内分泌激素，影响精子生成，最终导致精子数量和活力的下降。Objective To explore the correlation of the serum Leptin level of obsen male and semen parameters, as well as endocrine hormones level related to male sterility. Methods Total of 30 patients （the obesity group） and 10 healthy men （the control group） were enrolled in the study. The CASA was used to analyze semen quality of the patients. The levels of serum endocrine hormone were measured by Chemiluminescence, and the level of serum Leptin was detected by ELISA. Results The sperm concentration and a＋b grade sperm motility percentage in the control group were significantly higher than that in the obesity group （P〈0.01）. Levels of FSH, LH and T in the control group were significantly lower than those in the obesity group（P〈0.05）. The level of serum Leptin in the control group was significantly higher than that in the obesity group（P〈0.01）. The level of serum Leptin was significantly negatively correlated with sperm concentration and the levels of FSH、LH and T. Conclusion There are significant correlation between serum Leptin level and sperm parameters, as well as endocrine hormones level. Leptin may inhibit endocrine hormones and affect sperm production, then resulting in the decline of sperm number and motility.
