不同年龄组支气管哮喘患者急性发作与呼吸道感染的关系

摘　　要：目的 探讨不同年龄组病原体与哮喘急性发作的关系,为临床治疗提供依据。方法 对于2012年10月-2013年4月在海口市人民医院呼吸内科就诊的153例哮喘急性发作患者,依年龄分为青年组（18-40）岁72例,占47.1%,中老年组（41-70）岁81例,占52.9%,进行呼吸道病原九联检和痰培养并对结果进行统计分析。结果 153例患者中,有78例检出非细菌性病原体,阳性率为51.0%,其中青年组检出62例,阳性率为86.1%,中老年组检出16例,阳性率为19.8%,两组比较差异有统计学意义（P〈0.05）。分离出菌株78株,青年组检出菌株11株,占14.10%,中老年组检出菌株67株,占85.90%,两组比较差异有统计学意义（P〈0.05）。结论 不同年龄组哮喘急性发作的病原体不相同,青年组以非细菌病原体为主,中老年组以细菌为主,治疗时需选择合适的方案。Objective To analyze the relationship between acute asthma exacerbation and respiratory tract infection in different age groups so as to provide basis for the clinical treatment. Method A total of 153 patients with acute asthma exacerbation in respiratory department of Haikou Municipal People＇s Hospital from September 2012 to March 2013 were enrolled in this study. All the cases were divided into to groups according to age： young group（18-40 year,n=72,47.1%）,middle and elderly aged group（41-70year,n=81,52.9%）. The IgM to nine pathogens of respiratory tract and procalcitonin were tested and the pathogens of sputum were cultured. Result Out of 153 cases （51.0%） pathones were detected from 78, including 62 cases（86.1%） in the young group and 16 cases（19.8%） in the middle and elderly group, showing statistically significant difference between two groups （P〈0.05）. A total of 78 strains of micro-organisms were isolated from 153 patients, 11 strains in the young group （14.1%）, 67 strains （85.9%） in the middle and elderly group, being statistically significant different between them （P〈0.05）. Conolusion The pathogens of acute asthma exacerbation＇ s in different age groups were different. Non-bacterial infection could be detected in young group, bacterial infection in the middle old group, different therapeutic scheduls is suggested for treatment of case in different groups.
