摘  要:目的观察电针以及普通针刺对褥疮模型大鼠各种炎性细胞(肥大细胞、中性粒细胞、巨噬细胞、成纤维细胞)的影响,探讨电针在褥疮愈合过程中的作用机理。方法选取雄性SD大鼠75只,随机分为模型组、针刺组和电针组,每组25只。造模后模型组每日除进行基础护理外不做其他任何处理;针刺组每日基础护理同时进行针刺治疗;电针组每日基础护理同时先行针刺(同针刺组),随后连接直流电刺激仪进行电刺激治疗。每日治疗1次,直到取材时间点当天结束治疗。每组在治疗后第1、3、7、14、21 d时采用皮肤损伤面积追踪法,观察褥疮损伤面积的愈合情况,并观察不同时间点各种炎性细胞组织切片的组织学、形态学变化,同时各组进行肥大细胞计数。结果与模型组相比,针刺组大鼠在褥疮面积变化方面无显著性差异(P>0.05);电针组中褥疮面积平均值明显变小,具有显著性差异(P<0.05),且电针组大鼠在整个炎症反应过程中的各个时间点上,肥大细胞在致密度和脱颗粒程度上明显提前于模型组,中性粒细胞和巨噬细胞聚集出现的时间均早于模型组,随后出现的成纤维细胞聚集现象也提前于模型组。与针刺组相比,电针疗法可以明显降低褥疮面积的平均值,促使肥大细胞脱颗粒提前,具有显著性差异(P<0.05),并能加速中性粒细胞和巨噬细胞的聚集,使成纤维细胞聚集修复的时间提前。3组内部5个不同时间点比较,1褥疮面积方面,电针组面积平均值在治疗14 d时变化显著,与其余时间点比较具有显著性差异(P<0.05);模型组和针刺组面积平均值在治疗21 d时变化显著,与其余时间点比较具有显著性差异(P<0.05);2肥大细胞数目方面,电针组在治疗3 d时达到最高值,与其余时间点比较具有显著性差异(P<0.05);模型组和针刺组在治疗7 d时达到最高值,与其余时间点比较具有显著性差异(P<0.05)。结论电针傍�Objective To investigate the effects of electroacupuncture and conventional acupuncture on inflammatory cells in rat bedsore and explore the mechanism of electroacupuncture action in bedsore healing. Methods Seventy-five male SD rats were randomly allocated to model, acupuncture and electroacupuncture groups, 25 rats each. after model making, the model group received daily basic nursing alone; the acupuncture group, acupuncture treatment in addition to daily basic nursing; the electroacupuncture group, acupuncture (same as in the acupuncture group) followed by electrostimulation with a direct current stimulator in addition to daily basic nursing. Treatment was given once daily until the day of sampling time point. In every group, the bedsore healing area was observed using the skin lesion area tracing method at 1, 3, 7, 14 and 21 days after treatment. Histological and morphological changes on the section of tissue with various inflammatory cells were also observed at different time points. Mast cells were counted at the same time. Results Compared with the control group, the bedsore healing area did not change significantly in the conventional acupuncture group (P 〉 0.05) and its mean value decreased significantly in the electroacupuncture group (P〈0.05); mast cell density and degranulation, neutrophil and macrophage congregation, and subsequent fibroblast congregation occurred earlier in the electroacupuncture group. Compared with the acupuncture group, the mean value of bedsore healing area decreased significantly, mast cell degranulation occurred significantly earlier (P〈 0.05), neutrophil and macrophage congregation was accelerated and fibroblasts congregated and were repaired earlier in the electroacupuncture group. A comparison between five different time points in the three groups showed 1) the mean value of bedsore healing area changed significantly at 14 days after treatment compared with the other time points in the electroaeupuncture group (P〈0.05) and changed significantl
