血清脂连蛋白与超敏C反应蛋白在2型糖尿病合并冠心病诊断中的价值

摘　　要：目的探讨血清脂连蛋白与超敏C反应蛋白(hs-CRP)水平在2型糖尿病(T2DM)合并冠心病(CHD)诊断中的价值。方法选择2011年2月至2013年2月在新乐市医院门诊就诊或住院治疗的96例T2DM作为研究对象,其中48例合并CHD的患者作为T2DM-CHD组,48例单纯T2DM患者为T2DM组,另外选取同期在本院进行健康体检者50例作为对照组,测量并比较研究对象血清脂连蛋白、hs-CRP、空腹血糖(FBG)的水平。结果三组血清脂连蛋白、hs-CRP、FBG水平的比较,差异有统计学意义(F=3.681、4.256、3.470,P=0.016、0.007、0.034);其中T2DM-CHD组和T2DM组的脂连蛋白水平均低于对照组(q=7.603、4.028,P<0.05),hs-CRP和FBG水平显著高于对照组(P<0.05);T2DM组患者血清脂连蛋白水平显著高于T2DM-CHD组(q=3.752,P<0.05),hs-CRP、FBG水平显著低于T2DM-CHD(q=5.627、3.217,P<0.05),而且血清脂连蛋白水平与hs-CRP水平呈显著负相关(r=-0.692,P<0.05)。结论 T2DM患者血清中脂连蛋白、hs-CRP的水平能够反映CHD的严重程度,其对于T2DM并发CHD患者的诊断和治疗具有重要的临床价值。Objective To investigate the value of serum adiponectin and high sensitive C reactive protein（hs-CRP） in diagnosis of type 2 diabetes mellitus（T2DM） combined with coronary heart disease （CHD）.Methods Total of 96 patients with T2DM who visited the outpatient department or were hospitalized of Xinle Hospital from Feb.2011 to Feb.2013 were included as the objects,48 T2DM patients with combined CHD were selected as the T2DM-CHD group,and 48 patients with simple T2DM as the T2DM group,and 50 healthy people who received checkups in the hospital were selected as the control group,then the serum adiponectin,hs-CRP,fasting blood sugar （FBG）levels of the three groups were measured and compared.Results The serum adiponectin,hs-CRP,FBG levels of the three groups were statistically significantly different（F y 3.681,4.256,3.470,P y 0.016,0.007,0.034）.The adiponectin level of the control group was significantly higher than that of T2DM-CHD group and T2DM group （q y 7.603,4.028,P 〈 0.05）.hsCRP,FBG levels were significantly lower than that of T2DM group and T2DM group（P 〈 0.05）.The serum adiponectin level of T2DM group was higher than that of T2DM-CHD group （q y 3.752,P 〈 0.05）,hsCRP,FBG levels were lower than that of T2DM-CHD group（q y5.627,3.217,P 〈 0.05）,and the serum adiponectin level and hs-CRP level showed significant negative correlation（r =-0.692,P 〈 0.05）.Conclusion The serum adiponectin,hs-CRP levels of T2DM patients can reflect the severity of CHD,which has important clinical value on diagnosis and treatment of T2DM combined with CHD.
