热敏灸配合针刺在脑卒中后虚型便秘的临床应用及疗效观察

摘　　要：目的:观察热敏灸结合针刺治疗脑卒中后虚型便秘临床疗效。方法:将脑卒中后虚型便秘患者60例随机分为热敏灸治疗组30例和针刺对照组30例。针刺对照组和热敏灸治疗组均采用常规针刺治疗便秘外,热敏灸治疗组采用艾灸热敏腧穴方法治疗,两组均每日治疗1次,7次为1个疗程,共治疗2个疗程,除外两组病人均采用西医常规药物及改善肢体功能的针刺及功能康复治疗。2疗程后观察患者便秘症候评分、Bristol大便性状分型、近期疗效及疗程结束第10天观察远期疗效,并记录不良反应。结果:1热敏灸配合针刺治疗能显著改善中风后虚型便秘患者的便秘症状(5.22±2.68),疗效优于针刺对照组(6.35±3.05)(P<0.05);2热敏灸配合针刺治疗组治疗后26例患者的大便性状正常,优于针刺对照组(19例)(P<0.05).32个疗程治疗后,热敏灸配合针刺组的近期愈显率为63.33%,优于针刺组36.67%(P<0.05);4疗程结束后第15天,热敏灸组与针刺组的好转率分别为66.67%与53.33%,均能改善中风后虚型便秘症状,两者疗效无差异(P>0.05),但热敏灸配合针刺组的远期愈显率(46.67%),优于针刺组(20.00%)(P<0.05)。结论:热敏灸结合针刺法治疗卒中后虚型便秘疗效肯定,且操作简便、安全,值得临床推广。Objective： To observe the clinical effect of heat sensitive moxibustion on treating constipation of deficiency type after stroke. Method： 60 cases of patients with constipation of deficiency type after stroke were randomly divided into two groups： therapeutic group with 30 cases and controlled group with the same amount of cases. Except for the common routine acupuncture treatment on the two groups,the treatment group also received the heat sensitive moxibustion. The two groups were both treated once a day. Seven days made a course. The whole treatment included two courses. In additional to the above treatment,both of the groups were received the conventional medical treatment,acupuncture and rehabilitation to prove the sport function of affected limbs. After two courses,the two groups were compared in constipation symptom score,Bristol stool consistency typing,short term effect and long term effect which were observed on the tenth day since the end of the courses. Adverse reactions were recorded. Results：（1）Heat sensitive moxibustion and acupuncture can significantly prove the constipation symptoms of the patients with constipation of deficiency type after stroke（5. 22 ± 2. 68）,which had better effect than that of controlled group treated by acupuncture（6. 35 ± 3. 05）（ P〈0. 05）.（2）26 cases in therapeutic group got normal stool after the treatment,superior to the controlled group（19 cases）（ P〈0. 05）.（3）After two courses of treatments,the cure and significant effect rate of the therapeutic group was 63. 33%,which was better than that of the controlled group 36. 67%（ P〈0. 05）.（4） On the 15 thday after the courses,the recover rates of the therapeutic group and the controlled group were 66. 67% and 53. 33%. The two kinds of methous can prove the constipation symptoms of the patients with constipation of deficiency type after stroke. There was no difference between the effects of the two groups（ P〈0. 05）,but the long term cure and significant effect rate of
