小儿慢性咽喉炎与胃食管反流的关系

摘　　要：目的分析小儿慢性咽喉炎与胃食管反流(GER)的关系。方法胃食管反流症患儿400例,其中合并慢性咽喉炎患儿162例,随机进行分组,单纯组患儿80例,对其进行单纯慢性咽喉炎治疗;综合组患儿82例,进行胃食管反流和慢性咽喉炎的综合治疗,对发生胃食管反流情况进行分析。结果 400例小儿胃食管反流症发生小儿咽喉炎的发病率为40.50%(162/400),综合组患儿咽喉炎的临床治疗效果、症状缓解时间、内镜检查结果、患儿满意度等均明显优于单纯组患儿,差异具有统计学意义(P<0.05)。两组患儿均未发生因内镜检查、用药治疗导致的严重不良后果和不适症状。结论小儿胃食管反流症合并小儿慢性咽喉炎的比例较高,单纯进行小儿慢性咽喉炎的治疗,临床效果较差,综合性治疗小儿慢性咽喉炎和胃食管反流症可获得较好的临床治疗效果,在进行慢性咽喉炎的治疗中应注意胃食管反流症的诊断治疗,综合治疗为理想的治疗方法。Objective To analyze the relationship between pediatric chronic pharyngolaryngitis and gastroesophageal reflux （GER）. Methods There were 400 cases of GER with 162 cases combined with pediatric chronic pharyngolaryngitis. The cases were randomly divided into two groups. Simple group （n=80） received chronic pharyngolaryngitis treatment, and comprehensive group （n=82） received comprehensive treatment of pediatric chronic pharyngolaryngitis and GER. The occurrence of GER was analyzed. Results Among the 400 cases of children with GER, the morbidity of pediatric pharyngitis was 40.50% （162/400）. The clinical therapeutic effects of pharyngolaryngitis, relieve symptoms, endoscopic examination results and patient satisfaction of the comprehensive group were significantly better than those of the simple group. The differences were statistically significant （P〈0.05）. There were no severe adverse reactions and symptoms in the two groups caused by endoscopic examination and drugs. Conclusion The incidence of children with GER and pediatric chronic pharyngolaryngitis is high, therefore simple treatment of pediatric chronic pharyngolaryngitis has poor clinical effect. The comprehensive treatment of pediatric chronic pharyngolaryngitis and GER can provide good clinical effect. In the treatment of chronic pharyngolaryngitis, the diagnosis and treatment of GER should be noticed. The comprehensive treatment is an ideal method.
