肝脏淋巴瘤:薄层增强MRI多期扫描的诊断价值

摘　　要：目的探讨肝脏淋巴瘤的MRI表现。方法回顾性分析8例肝脏淋巴瘤的MRI表现,MRI扫描包括T1WI、T2WI、薄层增强三期扫描(动脉期、门静脉期、延迟期)。结果 8例肝淋巴瘤共18个病灶,12个病灶见门静脉紧贴或穿行。MRI T1WI呈等信号(2个)或稍低信号(16个),T2WI呈等信号(1个)或稍高信号(17个)。增强扫描时,4个病灶无强化,14个病灶弱强化(低于肝脏信号)、呈"慢进慢出"强化表现,其中10个强化病灶边缘见环形强化(高于肝实质信号)、呈"快进慢出"强化表现。3个病灶内见小裂隙状坏死。3例肝实质对比剂灌注不均。结论肝脏淋巴瘤主要分布于门静脉周围,大多数病灶T1WI呈稍低信号、T2WI呈稍高信号,呈无强化或弱强化表现,弱强化病灶边缘常有强化环包绕。病灶内常有正常血管穿行,坏死少见。Objective To investigate MRI findings of hepatic lymphoma, and to evaluate thin-slice enhanced dynamic muhiphasic MRI scanning in diagnosing hepatic lymphoma. Methods MRI findings in 8 cases with hepatic lymphoma were retrospectively analyzed. MRI scanning protocols included routine axial T1 WI, T2WI, and thin-slice dynamic contrast- enhanced tri-phasic sequence （arterial, portal venous and delayed phase）. Results A total of 18 lesions were detected in 8 patients. Twelve lesions were located close to the portal vein. The lesions exhibited isointensity （ n = 2） or hypointense （n = 16） on T1WI, while isointense （n = 1 ） or hyperintense （n = 17 ） on T2 WI. After contrast administration, 4 lesions showed no enhancement, 14 lesions had mild enhancement, presenting as “slow filling and slow washout” enhancement pattern. Of the 14 lesions showing enhancement, peripheral ring-shaped enhancement was observed in 10 with“fast filling and slow washout” sign. Small fissure-shaped necrosis was demonstrated in 3 lesions. Inhomogeneous enhancement of he- patic parenehyma was noted in 3 cases. Conclusion Most lesions of hepatic lymphoma are located around portal vein. The lesions are characterized by hypointense signal on T1 W and hyperintense signal on T2WI with no enhancement or only mild enhancement. Peripheral ring-shaped enhancement is often seen in the lesions that show mild enhancement. Within the lesions normal blood vessels are a common finding and necrosis is less seen.
