摘 要:目的探讨乳糜血对ALT测定的干扰程度,以及聚乙二醇(PEG)的预处理效果。方法从2012年10月~2013年3月血液常规筛查中随机收集甘油三酯(TG)正常的标本50份,其中30份用于配制不同浓度模拟脂血进行TG和ALT检测,用10%PEG处理不同浓度的模拟脂血并检测ALT。用不同浓度的PEG工作液对澄清组20份进行预处理并检测ALT。制备高、低值ALT混合血浆各11份分别进行PEG预处理方法的重复性评价。对临床检测中收集的乳糜血标本90例用10%PEG进行预处理并测定ALT,并与收集时的处理前结果进行比较,评价实际应用。结果低浓度脂血对ALT检测无明显影响,脂血浓度为5%时对ALT检测产生严重干扰。不同浓度的PEG处理乳糜血效果不同,尤以10%PEG处理效果为最佳。PEG处理法的不精密度均〈15%。90例标本处理后ALT值均在正常参考区间内。结论当模拟脂血浓度达到5%以上时,对ALT检测产生严重干扰。10%PEG可有效消除脂血对ALT检测的影响,处理方法的重复性也符合临床常规试验对精密度的要求。因此,PEG预处理可推荐作为实验室常规筛查方法。Objective To investigate the interference of hyperlipidemia samples on the detection of ALT,and to evaluate the effect of polyethylene glycol(PEG) pretreatment. Methods 50 blood samples with a normal level of triglycerides(TG) were collected randomly between October 2012 and March 2013, of which 30 samples were used in the preparation of different lipid concentrations and underwent detection tests for ALT and TG, with the addition of 10% PEG pretreatment. Another 20 samples, treated by different concentrations of PEG, were tested for ALT values. 11 mixed plasma of high ALT values and 11 with low ALT values were prepared in orderto evaluate the repeatability and imprecision of PEG pretreatment, respectively.From clinical tests, 90 lipid samples were selected randomly, were added 10% PEG pretreatment, and were detected for ALT concentrations,which were compared to the ALT levels in the san,pies without the addition of pretreatment. Results Low lipidconcentrations in samples had no significant effect on ALT detection, but lipid concentrations of more than 5% had serious in- terference on ALT detection. Different concentrations of PEG pretreatment had different outcomes, especially 10% PEG pre- treatment was considered the most ideal. The imprecision of PEG pretreatment method was reported to be less than 15%. The ALT concentrations of 90 lipid samples pre-treated by PEG were found in the normal reference range. Conclusion When the lipid concentration reached to 5% or more, ALT detection would be interfered. The results suggested that 10% PEG could effectively eliminate the interference and the repeatability of the method could meet the precision requirements of routine clinical tests. Therefore,PEG pretreatment could be recommended as a routine screening test.
