舒适护理对胫骨平台骨折术后老年患者心理状态、膝关节功能及生活质量的影响

摘　　要：目的探讨舒适护理对胫骨平台骨折术后老年患者心理状态、膝关节功能及生活质量的影响。方法选择110例老年胫骨平台骨折患者分为干预组和对照组各55例,干预组在对照组常规术后护理基础上实施舒适护理,观察两组焦虑和抑郁程度、疼痛和膝关节功能恢复效果及生活质量。结果干预组SAS评分、SDS评分显著低于对照组(P<0.05),焦虑程度、抑郁程度明显较对照组轻(P<0.05)。干预组第1天、7天、出院前NRS疼痛评分均显著低于对照组(P<0.05),干预组HHS评分显著高于对照组(P<0.05),干预组膝关节功能康复效果显著优于对照组(P<0.05)。干预组出院前SF-36量表生理功能、社会功能、生理职能、躯体疼痛、精神健康、情感功能、活力、总体健康等维度评分均显著高于对照组(P<0.05)。结论实施舒适护理可明显增加老年胫骨平台骨折患者术后舒适度,有效改善心理状态,减轻术后疼痛,有助于提高患者膝关节功能康复效果和生活质量。Objective To investigate the impact of comfort care for mentality, knee joint function and quality of life in elderly patients with tibial plateau fracture surgery. Methods A total of 110 cases of tibial plateau fractures in elderly patients were divided into intervention group and control group 55 cases, the intervention group received comfort care, anxiety and depression levels were observed , pain and knee function in the control group, based on the recovery rou- tine postoperative care effects and quality of life. Results The intervention group SAS score, SDS score were signifi- cantly lower than the control group （P〈0.05）, anxiety and depression degree were significantly lighter than the control group （P〈0.05）. Intervention group one day , seven days before discharge NRS pain scores were significantly lower than the control group （P〈0.05）, the intervention group was significantly higher HHS Score （P〈0.05）, knee rehabilitation re- sults intervention group was significantly superior than the control group （P〈0.05）. Before discharged the intervention group was SF-36 scale physiological function, social function , role-physical , bodily pain , mental health , emotional functioning, vitality, general health dimension scores were significantly higher （P〈0.05）. Conclusion The implementation of comfort care significantly increased postoperative comfort tibial plateau fractures in elderly patients , improve mental state, reduce postoperative pain, knee rehabilitation can help improve patient outcomes and quality of life.
